【摘要】：目的:分析真菌球型蝶竇炎的臨床特點及其與真菌球型上頜竇炎的差異。方法:回顧性分析50例經病理確診的真菌球型蝶竇炎病例(病例組)及同期273例真菌球型上頜竇炎病例(對照組)的臨床資料,比較其發(fā)病相關因素及臨床特點。結果:真菌球型蝶竇炎多見于50歲左右女性,左側較右側常見,病程在半年內的患者約占40%;主要癥狀為頭痛、鼻塞、涕中帶血、異味涕、眼脹、鼻根脹痛、溢淚等。與真菌球型上頜竇炎相比,蝶竇炎頭痛、眼脹、溢淚更常見(P0.05)。CT表現(xiàn)為病變竇腔軟組織影中有鈣化影,可伴局部骨質的增厚、硬化、缺損。50例真菌性蝶竇炎中僅1例有二次手術史,手術治愈率高達98%。結論:真菌球型蝶竇炎臨床癥狀多樣,頭痛、眼脹、溢淚具有相對特異性。CT診斷特異性較強,手術是最有效的治療方法。
[Abstract]:Objective: to analyze the clinical characteristics of fungal spherical sphenoid sinusitis and its difference from fungal spherical maxillary sinusitis. Methods: the clinical data of 50 cases of fungal sphenoid sinusitis diagnosed by pathology and 273 cases of fungal bulbar maxillary sinusitis (control group) were analyzed retrospectively. Results: fungal spherical sphenoid sinusitis was more common in females at the age of 50 years, the left side was more common than the right side, and the course of disease was about 40 in half a year. The main symptoms were headache, nasal congestion, blood in snot, bad smell, eye distension, distension of nose root, overflowing tears and so on. Compared with fungal bulbar maxillary sinusitis, sphenoid sinusitis is more common in headache, eye distention, and lacrimal overflow (P0.05). Ct features include calcification in soft tissue shadow of diseased sinus cavity, which can be accompanied by thickening and sclerosis of local bone. Of the 50 cases of fungal sphenoid sinusitis, only 1 case had a history of secondary surgery, and the cure rate was as high as 98%. Conclusion: the clinical symptoms of fungal sphenoid sinulitis are various, headache, eye distension, and lacrimal sphenoid sinulitis have relative specificity. Ct is the most effective method for the diagnosis of sphenoid sinusitis.
