【摘要】：目的探討兒童特發(fā)性視神經炎的臨床特點,評估鼠神經生長因子(NGF)聯合甲基強的松龍沖擊治療兒童特發(fā)性視神經炎的療效。方法回顧性分析2014年1月~2015年4月我院門診及病房確診的62例(62只眼)兒童特發(fā)性視神經炎患兒的臨床資料及隨訪資料。結果兒童視神經炎發(fā)病急,視力下降快,視野、VEP均有不同程度損害。NGF聯合甲基強的松龍沖擊治療(治療組)及單純甲基強的松龍沖擊治療(對照組)后患眼的視力、視野、P-VEP均有不同程度改善。治療一周后治療組總有效率為93.55%,對照組為80.65%,治療組療效優(yōu)于對照組(P0.05);治療3個月后,兩組的總有效率無明顯差異(P0.05)。治療前后兩組的神經纖維層厚度均變薄,顳側和鼻側尤為明顯(P0.05)。結論對于兒童特發(fā)性視神經炎,早期聯合應用鼠神經生長因子和甲基強的松龍沖擊治療是一種有效的方法。VEP和OCT在該病的臨床研究中有重要的應用價值。
[Abstract]:Objective to investigate the clinical features of idiopathic optic neuritis in children and to evaluate the efficacy of NGF combined with methylprednisolone in the treatment of idiopathic optic neuritis in children. Methods the clinical data and follow-up data of 62 children with idiopathic optic neuritis from January 2014 to April 2015 were retrospectively analyzed. Results the visual acuity of children with optic neuritis was acute, visual acuity was decreased rapidly, visual field VEP was damaged in different degree. NGF combined with methylprednisolone shock therapy (treatment group) and simple methylprednisolone impact therapy (control group). Visual field P-VEP was improved to some extent. One week after treatment, the total effective rate of the treatment group was 93.55 and that of the control group was 80.65. The curative effect of the treatment group was better than that of the control group (P 0.05), and after 3 months of treatment, there was no significant difference in the total effective rate between the two groups (P 0.05). Before and after treatment, the thickness of nerve fiber layer in both groups became thinner, especially in temporal side and nasal side. Conclusion early combination of nerve growth factor and methylprednisolone is an effective method for the treatment of idiopathic optic neuritis in children. VEP and Oct have important application value in the clinical study of the disease.
