【摘要】：目的:運(yùn)用間歇性外斜視生存質(zhì)量量表(Intermittent Exotropia Questionnaire,IXTQ)評(píng)估并比較間歇性外斜視(intermittent exotropia,IXT)兒童與正常兒童生存質(zhì)量(quality of life,QOL),并通過IXTQ評(píng)分研究IXT兒童及父母生存質(zhì)量與斜視度的關(guān)系。方法:運(yùn)用IXTQ量表對(duì)研究對(duì)象(間歇性外斜視及正常兒童各68例,及其父母)進(jìn)行問卷調(diào)查。包括:評(píng)估患兒生存質(zhì)量的患兒自評(píng)表(Child IXTQ)及父母代評(píng)表(Proxy IXTQ),以及評(píng)估父母生存質(zhì)量的父母自評(píng)表(Parent IXTQ),其中患兒自評(píng)表和父母代評(píng)表各包括視覺功能和社會(huì)心理兩個(gè)維度,父母自評(píng)表包括視覺功能、社會(huì)心理和手術(shù)三個(gè)維度。比較斜視組與正常對(duì)照組IXTQ評(píng)分的差異。斜視組兒童采用遮蓋單眼1小時(shí)后三棱鏡加交替遮蓋法檢查視遠(yuǎn)(5m)及視近(33cm)斜視度,并分為3組:20≤PD40、40≤PD60、PD≥60,研究IXTQ評(píng)分與不同斜視度的關(guān)系。結(jié)果:1.斜視組Child IXTQ、Proxy IXTQ及Parent IXTQ評(píng)分均低于正常對(duì)照組。Child IXTQ評(píng)分中與視覺功能相關(guān)的條目:如強(qiáng)光下閉一眼(62.50±32.46 vs85.61±24.04)、擔(dān)心眼睛(51.84±27.08 vs 70.83±31.19)有顯著性差異(P0.05),而與社會(huì)心理相關(guān)的條目:如嘲笑、交友困難、擔(dān)心別人的看法差別無統(tǒng)計(jì)學(xué)意義;Parent IXTQ評(píng)分中視覺功能(49.21±23.13 vs 90.12±18.83)、社會(huì)心理(59.97±22.15vs 91.02±15.31)及手術(shù)維度(52.02±28.12 vs 86.74±15.48)均有顯著性差異(P0.05)。2.不同視遠(yuǎn)斜視度組中,Proxy IXTQ及Parent IXTQ評(píng)分存在顯著性差異(F3.416,P0.039;F4.197,P0.019),而Child IXTQ評(píng)分差異無統(tǒng)計(jì)學(xué)意義;不同視近斜視度組中,Proxy IXTQ及Parent IXTQ評(píng)分也存在顯著差異(F6.086,P0.004;F3.295,P0.043),而Child IXTQ評(píng)分差異無統(tǒng)計(jì)學(xué)意義。3.Child IXTQ評(píng)分與視遠(yuǎn)、視近斜視度無明顯相關(guān)性,Proxy IXTQ評(píng)分與視遠(yuǎn)、視近斜視度均存在負(fù)相關(guān)(r-0.274,P0.024;r-0.325,P0.007),Parent IXTQ評(píng)分與視遠(yuǎn)、視近斜視度也均存在負(fù)相關(guān)關(guān)系(r-0.324,P0.007;r-0.251,P0.039),其中社會(huì)心理維度評(píng)分與視遠(yuǎn)斜視度存在負(fù)相關(guān)(r-0.314,P0.009),手術(shù)維度評(píng)分與視遠(yuǎn)、視近斜視度均成負(fù)相關(guān)(r-0.435,P0.001;r-0.420,P0.001)。結(jié)論:1.間歇性外斜視兒童及父母生存質(zhì)量均低于正常兒童及父母,其中影響兒童生存質(zhì)量的主要是對(duì)視覺功能方面的擔(dān)憂,而對(duì)社會(huì)心理方面擔(dān)憂小,影響父母生存質(zhì)量的主要是對(duì)孩子視覺功能、社會(huì)心理及手術(shù)方面的擔(dān)憂。2.間歇性外斜視兒童生存質(zhì)量與斜視度關(guān)系不大,但父母生存質(zhì)量與斜視度成負(fù)相關(guān),斜視度增大,父母生存質(zhì)量下降,主要體現(xiàn)在對(duì)孩子手術(shù)方面的擔(dān)憂。3.建議將IXTQ評(píng)估生存質(zhì)量做為IXT兒童及父母的常規(guī)檢測(cè)項(xiàng)目,以指導(dǎo)臨床個(gè)體化治療。
[Abstract]:Objective: to evaluate and compare the quality of life (QOL) between children with intermittent exotropia and normal children with intermittent Exotropia questionnaire IXTQs, and to study the relationship between the quality of life (QOL) and strabismus in children with IXT and their parents by means of IXTQ score. Methods: the subjects (68 cases of intermittent exotropia and 68 cases of normal children, and their parents) were investigated with IXTQ scale. These include Child IXTQA (Child IXTQ1), proxy IXTQ1 (parent substitute), and parent IXTQ1 (parent quality of Life), which include visual function and psychosocial dimensions respectively.
