【摘要】：目的研究絲裂霉素C-殼聚糖(MMC-CS)緩釋微球?qū)ν们喙庋蹫V過術(shù)區(qū)瘢痕增殖的療效。方法 48只(96眼)新西蘭大白兔分為4組,每組24眼,通過雙眼行鞏膜咬切術(shù)建立青光眼濾過術(shù)兔模型。其中24只兔的右眼術(shù)中植入MMC-CS緩釋微球(A組),其左眼術(shù)中植入CS空白緩釋微球(B組);另24只兔右眼術(shù)中應用含0.20mg/mL MMC棉片(C組),其左眼行單純鞏膜咬切術(shù)(D組)。術(shù)后觀察各組眼壓、濾過泡、前房炎性反應、并發(fā)癥、角膜內(nèi)皮細胞計數(shù)等情況;術(shù)后第7、14、21天分批處死各組兔模型并行病理學檢查。結(jié)果 (1)眼壓:A組能維持較長時間的低水平眼壓,C組次之,B組與D組維持時間最短,組間差異有統(tǒng)計學意義(P0.01)。(2)濾過泡情況:A組濾過泡存活時間較其他各組長,差異有統(tǒng)計學意義(P0.01)。(3)并發(fā)癥:各組中均未出現(xiàn)術(shù)后并發(fā)癥。(4)角膜內(nèi)皮細胞計數(shù):A組術(shù)前與術(shù)后的計數(shù)值差異無統(tǒng)計學意義(P0.05)。(5)病理結(jié)果:A組結(jié)膜上皮完整性較好,結(jié)膜下成纖維細胞較其他各組少,濾過口周圍區(qū)無明顯炎癥浸潤,新生膠原纖維也較少。結(jié)論 MMCCS緩釋微球能安全有效抑制兔青光眼濾過術(shù)區(qū)的成纖維細胞及新生膠原纖維的增殖,顯著提高青光眼濾過術(shù)的成功率。
[Abstract]:Objective to study the effect of mitomycin C-chitosan MMC-CSC microspheres on scar proliferation in rabbit glaucoma filtration operation. Methods Forty-eight New Zealand white rabbits were divided into 4 groups with 24 eyes in each group. The rabbit model of glaucoma filtration was established by scleral bite in both eyes. MMC-CS sustained release microspheres were implanted into the right eyes of 24 rabbits during operation, while CS blank sustained release microspheres were implanted in the left eye in group B, while the right eyes of 24 rabbits were treated with 0.20mg/mL MMC cotton film in group C and group D with simple scleral bite in their left eyes. Intraocular pressure, filtering bleb, anterior chamber inflammatory reaction, complications and corneal endothelial cell count were observed after operation. Results (1) the intraocular pressure (IOP) group A could maintain low level IOP for a long time, the second group was group B and group D was the shortest, and the difference was statistically significant (P < 0.01). The survival time of filtering bleb in group A was longer than that in group C, and the survival time of filtering bleb in group A was longer than that in group C (P < 0.01), and the survival time of bleb in group A was longer than that in group C. There was no significant difference in corneal endothelial cell count between group A and group A before and after operation. There was no significant difference in the count of corneal endothelium between group A and group A (P 0.05). The pathological results showed that the integrity of conjunctival epithelium in group A was better than that in group A (P < 0.05). The number of subconjunctival fibroblasts was less than that of other groups. Conclusion MMCCS sustained release microspheres can effectively and safely inhibit the proliferation of fibroblasts and new collagen fibers in rabbit glaucoma filtering area, and improve the success rate of glaucoma filtration surgery.
