84例唇腭裂患者聽覺失匹配負波的研究

【摘要】:目的對純音測聽(pure tone audiometry,PTA)、聽性腦干反應(auditory brain stem response,ABR)正常的唇腭裂患者行聽覺失匹配負波(auditory mismatch negativity,AMMN)檢查,并與健康正常人對照,判斷唇腭裂患者聽覺皮層的功能狀態(tài),為探討其言語功能障礙機制提供理論依據(jù)。方法唇裂伴有腭裂(Cleft Lip and Palate,CLP)39例,單純唇裂(Cleft Lip,CL)28例,單純腭裂(Cleft Palate,CP)17例,健康志愿者(正常組)39例(純音測聽、聽性腦干反應均正常)分別行AMMN檢查,比較各組間潛伏期及波幅的差異。結果 CP組AMMN潛伏期較正常組、CL組顯著延長(P0.05),CP組潛伏期較CLP組無明顯差異(P0.05);CLP組潛伏期較CL組、正常組顯著延長(P0.05);CL組潛伏期與正常組無明顯差異(P0.05)。各組間AMMN的波幅無明顯差異(P0.05)。結論 CP及CLP患者聽覺皮層的功能可能存在異常,可能為導致其言語功能障礙的一個原因,但仍然需要結合其言語識別能力、言語功能評定、磁共振(magnetic resonance,MR)、功能性磁共振(functional magnetic resonance imaging,fMRI)加以進一步研究。
[Abstract]:Objective to evaluate the functional status of auditory cortex in patients with normal auditory brainstem response (ABR), pure tone audiometry and auditory brainstem response (ABR). Methods the patients with cleft lip and palate were examined by auditory mismatch negative mismatch (AMMNs), and compared with those of healthy controls, and to determine the functional status of auditory cortex in patients with cleft lip and palate, and to determine the function of auditory cortex in patients with cleft lip and palate, and to evaluate the function of auditory cortex in patients with cleft lip and palate. To provide a theoretical basis for exploring the mechanism of speech dysfunction. Methods AMMN examination was performed in 39 patients with cleft Lip and palatography, 28 patients with cleft and 17 patients with cleft palatal cleft palsy. 39 normal subjects (pure tone audiometry and normal auditory brainstem response) were examined by AMMN. The latency and amplitude of each group were compared. Results the latent period of AMMN in CP group was significantly longer than that in control group. There was no significant difference in latency of CP group compared with that of CLP group, but there was no significant difference between normal group and normal group. There was no significant difference in the amplitude of AMMN among groups (P 0.05). Conclusion the abnormal function of auditory cortex may be one of the causes of speech dysfunction in CP and CLP patients, but it is still necessary to evaluate the speech function in combination with their speech recognition ability. Magnetic resonance and functional magnetic resonance imaging of MRI were further studied.
