【摘要】：目的： 通過測定鼻咽癌患者放療前后垂體激素水平,探討放療對鼻咽癌(NPC)患者的垂體激素水平的影響。 方法： 從2003年8月至2011年8月,在浙江大學附屬二院腫瘤放療科一個治療小組選擇101例鼻咽癌患者,行回顧性描述性研究。對這101例鼻咽癌患者,行鼻咽部及頸部淋巴結放療前及放療結束時、放療后數(shù)月至數(shù)年間,復查其血清中高敏促甲狀腺激素(h-TSH)、生長激素(GH)、催乳素(PRL)、黃體生成素(LH)、卵泡刺激素(FSH)的濃度,通過統(tǒng)計分析,探討放療對鼻咽癌(NPC)患者的垂體激素水平的是否有影響,對何種垂體激素影響更明顯,何時出現(xiàn)最明顯的影響等。 結果： 總病例共101例,其中男性64例,女性37例,男女比例為：1.81：1.年齡在16-83歲之間,中位年齡為49歲。對101例患者放療前及放療結束時、放療后數(shù)月至數(shù)年間,復查其血清中生長激素(GH)、催乳素(PRL)、黃體生成素(LH)、卵泡刺激素(FSH)及促甲狀腺激素(TSH)的濃度,從放療前開始入組,最長隨訪至放療后73月。結果顯示高敏促甲狀腺激素(h-TSH)在放療后4月-4年間激素水平變化存在統(tǒng)計學差異(P0.05)；生長激素(GH)在放療后1-3月及7-12月較放療前激素水平變化存在統(tǒng)計學差異(P0.05)；男性催乳素(PRL)在放療后7-12月較放療前激素水平變化存在統(tǒng)計學差異(P0.05)；男性黃體生成素(LH)在放療后4-6月較放療前激素水平變化存在統(tǒng)計學差異(P0.05),而在放療后19-24月內較放療結束時黃體生成素(LH)激素水平仍有統(tǒng)計學差異(P0.05)；男性卵泡刺激素(FSH)在放療后3年內較放療前激素水平變化有統(tǒng)計學差異(P0.05)。 結論： 鼻咽癌患者放療前后垂體激素水平發(fā)生了動態(tài)的變化。
[Abstract]:Objective:To investigate the effect of radiotherapy on pituitary hormone levels in patients with nasopharyngeal carcinoma (NPC) before and after radiotherapy.Methods:From August 2003 to August 2011, a retrospective descriptive study was conducted in 101 patients with nasopharyngeal carcinoma in the Department of tumor radiotherapy, second affiliated Hospital of Zhejiang University.For these 101 patients with nasopharyngeal carcinoma, the nasopharyngeal and cervical lymph nodes were treated months to years after radiotherapy, before and at the end of radiotherapy.To investigate the effects of radiotherapy on pituitary hormone levels in patients with nasopharyngeal carcinoma (NPC), the serum concentrations of Gao Min TSH, GH, PRL, LHN and FSHS were reviewed, and the effects of radiotherapy on pituitary hormone levels in patients with nasopharyngeal carcinoma (NPC) were investigated by statistical analysis.To which pituitary hormone influence is more obvious, when appears the most obvious influence and so on.Results:The total number of cases was 101, including 64 males and 37 females, with a ratio of 1. 81 to 1. 1.The age ranged from 16 to 83 years, with a median age of 49 years.The serum concentrations of growth hormone GHG, prolactin prolactin, luteinizing hormone (LHH), follicle stimulating hormone (FSH) and thyroid stimulating hormone (TSH) were examined in 101 patients before and after radiotherapy for months to several years after radiotherapy.Up to 73 months after radiotherapy.The results showed that there were significant differences in hormone levels between 4 and 4 months after radiotherapy (P 0.05) and between January and July in July and December after radiotherapy. There were significant differences in hormone levels between January and January and July and December after radiotherapy in males; in males, there were significant differences in hormone levels between 3 months and 7 months after radiotherapy.Prolactin prolactin (PRL) levels were significantly different from those before radiotherapy in July and 12 months after radiotherapy (P 0.05), and in male luteinizing hormone (LH) from April to June after radiotherapy compared with those before radiotherapy (P 0.05), and within 19 to 24 months after radiotherapy, there was a significant difference in the changes of hormone levels in male luteinizing hormone (LH).At the end of the therapy, there was still a statistical difference in the level of luteinizing hormone (LH) and the changes of male follicle stimulating hormone (FSH) in three years after radiotherapy compared with that before radiotherapy (P 0.05).Conclusion:Pituitary hormone levels in nasopharyngeal carcinoma patients changed dynamically before and after radiotherapy.
