【摘要】：目的:探討腫瘤相對(duì)體積對(duì)接受同步調(diào)強(qiáng)放化療的局部晚期鼻咽癌患者的預(yù)后影響。材料與方法:2009年1月1日至2012年12月30日期間經(jīng)廣西醫(yī)科大學(xué)第一附屬醫(yī)院放療科病理確診的初治鼻咽癌的患者143例入組本組研究。所有患者均為III IVb期(UICC分期),均接受調(diào)強(qiáng)放療+同步化療。腫瘤體積由計(jì)劃系統(tǒng)三維重建并自動(dòng)計(jì)算�？偰[瘤相對(duì)體積(RGTV)=總腫瘤體積(GTV)/人體體積,原發(fā)鼻咽部腫瘤相對(duì)體積(RGTVnx)=原發(fā)鼻咽部腫瘤體積(GTVnx)/人體體積,轉(zhuǎn)移淋巴結(jié)相對(duì)體積(RGTVnd)=轉(zhuǎn)移淋巴結(jié)體積(GTVnd)/人體體積。結(jié)果:RGTV、RGTVnx,RGTVnd的截點(diǎn)分別為2.38、1.091、0.273。RGTV2.38 vs RGTV≤2.38兩組患者5年總生存率、無(wú)瘤生存率、無(wú)遠(yuǎn)處轉(zhuǎn)移生存率、無(wú)復(fù)發(fā)生存率分別為46.1%與95.8%(P0.001),42.5%與97.0%(P0.001)、52.8%與98.0%(P0.001),72.7%與98.0%(P0.001)。RGTVnx1.091與RGTVnx≤1.091兩組患者5年總生存率、無(wú)病生存率、無(wú)遠(yuǎn)處轉(zhuǎn)移生存率、無(wú)復(fù)發(fā)生存率分別為65.4%與93.4%(P0.001),65.2%與94.8%(P0.001)、71.4%與97.4%(P0.001),84.8%與97.3%(P=.003),RGTVnd0.273與RGTVnd≤0.273兩組的患者的5年總生存率、無(wú)病生存率、無(wú)遠(yuǎn)處轉(zhuǎn)移生存率、無(wú)復(fù)發(fā)生存率分別為70.3%與96.5%(P0.001),70.1%與94.8%(P0.001),77.5%與98.2%(P0.001),88.5%與96.6%(P=0.049)。結(jié)論:腫瘤相對(duì)體積是鼻咽癌的獨(dú)立預(yù)后因素。
[Abstract]:Objective: to investigate the effect of tumor relative volume on prognosis of locally advanced nasopharyngeal carcinoma (NPC) patients undergoing synchronous intensity modulated radiotherapy and chemotherapy.Materials and methods: from January 1, 2009 to December 30, 2012, 143 patients with nasopharyngeal carcinoma (NPC) diagnosed pathologically in the Department of radiotherapy of the first affiliated Hospital of Guangxi Medical University were enrolled in this study.All the patients were treated with intensity modulated radiotherapy (IMRT) combined with chemotherapy.Tumor volume is reconstructed by the three-dimensional planning system and calculated automatically.The relative volume of primary nasopharynx neoplasms is the volume of primary nasopharyngeal tumor, the volume of primary nasopharynx tumor is GTVnxX / human body volume, the relative volume of metastatic lymph node is RGTVndD = the volume of metastatic lymph node is GTVndT / human body volume, the relative volume of primary nasopharyngeal tumor is the same as the volume of primary nasopharynx tumor and the relative volume of metastatic lymph node is equal to the volume of metastatic lymph node.The recurrence free survival rate was 70.3% and 96.5%, respectively, and 70.1% and 94.8%, respectively, and 77.5% and 98.2% respectively.Conclusion: tumor relative volume is an independent prognostic factor for nasopharyngeal carcinoma.
