數(shù)值模擬鼻中隔偏曲伴棘突對鼻腔氣流的影響

【摘要】:目的:利用鼻中隔偏曲患者鼻腔的CT掃描數(shù)據(jù)建立鼻腔的CFD模型,并通過數(shù)值模擬的鼻腔流場進(jìn)行分析,觀察鼻中隔棘突對氣流的影響,判斷與鼻塞癥狀的關(guān)系。 方法:36例鼻中隔偏曲棘突患者采用視覺模擬量表評分(VAS)評價鼻塞癥狀嚴(yán)重程度。應(yīng)用16排螺旋CT對36例鼻中隔偏曲棘突型患者及30例正常人進(jìn)行鼻部的連續(xù)薄層掃描。應(yīng)用醫(yī)學(xué)圖象重建軟件Mimics導(dǎo)入CT掃描數(shù)據(jù)建立鼻腔三維模型,再轉(zhuǎn)換為CFD模型。通過計算機(jī)流體力學(xué)軟件Fluent設(shè)置在潮氣量400ml的條件下進(jìn)行模擬及再現(xiàn)鼻腔氣流流動。 結(jié)果:36例鼻中隔偏曲棘突患者,VAS評分為7(6-8)。經(jīng)過Fluent模擬潮氣量在400ml流量下鼻中隔棘突處吸氣及呼氣相峰流速分別為8.1±1.52m/s,-6.7±0.45m/s.棘突附近壓強(qiáng)降變化明顯,吸氣時棘突前方壓強(qiáng)升高,棘突后方出現(xiàn)壓強(qiáng)減低區(qū)域,呼氣時相反。VAS評分與吸氣及呼氣相峰流速及鼻中隔棘突周圍氣流速度及壓強(qiáng),棘突距前鼻棘距離等因素進(jìn)行Spearman等級相關(guān)性分析。VAS與上述指標(biāo)無明顯相關(guān)性。 結(jié)論:36例鼻中隔偏曲患者及30例正常人鼻腔的CFD模型順利建立。鼻中隔棘突患者呼吸氣流分布與正常人不同,棘突阻礙正常氣流流動,改變棘突附近流量分布,對鼻腔內(nèi)部壓強(qiáng)分布產(chǎn)生影響,但流速、壓力的改變與鼻塞癥狀無明顯相關(guān)性。
[Abstract]:Objective: to establish the CFD model of nasal cavity with CT scanning data of nasal septum deviation, and to observe the influence of spinous process of nasal septum on airflow and to judge the relationship between nasal obstruction and nasal obstruction by numerical simulation of nasal cavity flow field.Methods 36 patients with septum deviation and spinous process were evaluated by visual analogue scale (VASs).Continuous thin slice scan of nasal region was performed in 36 patients with septal deviation and spinous process and 30 normal subjects by 16-slice spiral CT.Medical image reconstruction software Mimics was used to import CT scanning data to establish a three-dimensional model of nasal cavity, and then converted to CFD model.The simulation and reappearance of nasal air flow were carried out by computer fluid dynamics software Fluent under the condition of tidal volume 400ml.Results the VAS score of 36 patients with septum deviation and spinous process was 76-8.The peak flow rate of inspiratory and expiratory phase at the spinous process of nasal septum after Fluent simulation was 8.1 鹵1.52 m / s -6.7 鹵0.45 m / s, respectively.The pressure drop in the vicinity of the spinous process was obvious, and the pressure in front of the spinous process increased during inspiratory period, and the area of decrease in pressure appeared behind the spinous process. On exhalation, the VAS score was opposite to the peak flow rate of inspiratory and expiratory phase and the velocity and pressure of airflow around the spinous process of nasal septum.The correlation analysis of Spearman grade and the distance between spinous process and anterior nasal spine was carried out. There was no significant correlation between Spearman and the above indexes.Conclusion the CFD model of nasal cavity of 36 patients with nasal septum deviation and 30 normal subjects was successfully established.The distribution of respiratory airflow in patients with spinous process of nasal septum is different from that in normal people. Spinous process obstructs the flow of normal airflow, changes the distribution of flow near spinous process and affects the distribution of internal pressure in nasal cavity, but the change of velocity and pressure has no significant correlation with nasal congestion symptoms.
