【摘要】：目的：探討西寧地區(qū)阻塞性睡眠呼吸暫停低通氣綜合征（OSAHS）與高血壓的關系及OSAHS相關高血壓的發(fā)病機制。 方法:選擇西寧地區(qū)33例OSAHS患者作為研究對象，均為男性，平均年齡46.91±9.03歲；20例健康人群作為健康對照組，均為男性，年齡40.80±12.14歲。兩組人員均在行多導聯(lián)睡眠監(jiān)護前及監(jiān)護后測定血壓，并將OSAHS患者的血壓與各睡眠呼吸參數(shù)進行相關分析。 結果：1）西寧地區(qū)OSAHS組體重指數(shù)（BMI）較正常組明顯升高(P0.05)；2）西寧地區(qū)OSAHS組呼吸暫停最長時間、呼吸暫停指數(shù)（AHI）、氧飽和度（SaO_2）85%時間均明顯高于對照組(P0.05),而OSAHS組的平均氧飽和度（MSaO_2）、最低氧飽和度（LSaO_2）均明顯低于對照組(P0.05)；3）西寧地區(qū)OSAHS組的睡前及醒后血壓均高于對照組(P0.05)；4）西寧地區(qū)OSAHS組的收縮壓（SBP）、舒張壓（DBP）與AHI呈正相關(P0.05)，與LSaO_2呈負相關(P0.05)。 結論：西寧地區(qū)OSAHS組的SBP、DBP與AHI呈正相關，與LSaO_2呈負相關，且OSAHS組LSaO_2較對照組明顯降低，提示OSAHS引起的夜間低氧血癥可能是高血壓病因之一，低壓低氧環(huán)境可能會進一步加重高血壓的發(fā)生。
[Abstract]:Objective: to investigate the relationship between obstructive sleep apnea hypopnea syndrome (OSAHS) and hypertension and the pathogenesis of OSAHS related hypertension in Xining area. Methods: 33 patients with OSAHS in Xining area were selected as study subjects, all of them were male, and 20 healthy people (mean age 46.91 鹵9.03 years old) were selected as healthy control group, all of them were male. The age was 40.80 鹵12.14 years old. The blood pressure was measured before and after polysomnography in both groups, and the correlation between blood pressure and sleep breathing parameters in OSAHS patients was analyzed. Results the body mass index (BMI) of the OSAHS group in Xining area was significantly higher than that of the normal group (P 0.05). The longest time of apnea was found in the OSAHS group in Xining area. Apnea index (AHI), oxygen saturation (SAO _ 2 / 85%) were significantly higher than those in control group (P 0.05), while the mean oxygen saturation (MSaO _ 2) and lowest oxygen saturation (L _ SaO _ 2) in OSAHS group were significantly lower than those in control group (P < 0.05). The blood pressure before sleep and after waking in OSAHS group in Xining area was higher than that in control group. The systolic blood pressure (SBP) and diastolic blood pressure (DBP) of OSAHS group in Xining area were positively correlated with AHI (P 0.05) and negatively correlated with LSaO_2 (P 0.05). Conclusion: SBP AHI in OSAHS group in Xining area was positively correlated with AHI and negatively correlated with LSaO_2, and LSaO_2 in OSAHS group was significantly lower than that in control group, suggesting that OSAHS induced nocturnal hypoxemia may be one of the causes of hypertension. Low-pressure hypoxia may further aggravate the occurrence of high blood pressure.
