【摘要】：目的探討頸內(nèi)動(dòng)脈眼動(dòng)脈段動(dòng)脈瘤血管內(nèi)栓塞治療方法及技術(shù)要點(diǎn)。方法回顧性分析2012年5月至2015年4月鄭州大學(xué)第一附屬醫(yī)院采用血管內(nèi)栓塞術(shù)治療48例眼動(dòng)脈段動(dòng)脈瘤患者影像及臨床資料、治療效果及隨訪結(jié)果,探討治療方法及技術(shù)要點(diǎn)。結(jié)果 48例患者49枚動(dòng)脈瘤均栓塞成功,其中單純彈簧圈栓塞17枚(雙微導(dǎo)管3枚),球囊輔助栓塞2枚,支架輔助栓塞28枚(Leo支架3枚,Lvis支架5枚,Enterprise支架14枚,Solatire AB支架6枚),Willis覆膜支架隔絕1枚,Pipeline密網(wǎng)支架植入1枚。術(shù)后即刻DSA造影顯示,Raymond栓塞分級(jí)Ⅰ級(jí)34枚(69%),Ⅱ級(jí)9枚(18%),Ⅲ級(jí)6枚(13%)。術(shù)中發(fā)生支架內(nèi)血栓形成2例。48例均達(dá)到臨床治愈,出院時(shí)改良Rankin量表(m RS)評(píng)分0分44例,1分2例,4分2例(該2例治療前≥4分);出院后3個(gè)月至1年m RS評(píng)分0分46例,2分1例,4分1例,均無(wú)新發(fā)神經(jīng)功能障礙及出血。20例DSA隨訪1~21個(gè)月,18例痊愈,1例明顯好轉(zhuǎn),1例明顯復(fù)發(fā),再次手術(shù)致密栓塞。結(jié)論血管內(nèi)栓塞治療眼動(dòng)脈段動(dòng)脈瘤是一種微創(chuàng)、安全有效的方法,選擇不同栓塞技巧是保證手術(shù)成功、減少術(shù)后并發(fā)癥、提高治愈率的關(guān)鍵。
[Abstract]:Objective to explore the methods and technical points of endovascular embolization of ophthalmic artery aneurysms of internal carotid artery. Methods from May 2012 to April 2015, 48 cases of eye movements were treated with endovascular embolization in the first affiliated Hospital of Zhengzhou University. Imaging and clinical data of patients with venous aneurysm, Results all 49 aneurysms of 48 patients were successfully embolized, of which 17 were embolized by simple coils (3 with double microcatheter and 2 with balloon assisted embolization).
