【摘要】：目的分析兒童先天性白內(nèi)障手術(shù)方式、人工晶狀體(intraocular lens,IOL)植入情況和術(shù)后并發(fā)癥情況,以期為兒童先天性白內(nèi)障的治療提供參考。方法回顧分析2008年1月至2016年1月在我院行先天性白內(nèi)障摘出的13歲以下兒童的臨床資料。手術(shù)前后行裂隙燈顯微鏡、眼底、眼壓、最佳矯正視力等常規(guī)檢查。2歲及以下兒童行白內(nèi)障超聲乳化摘出+后囊連續(xù)環(huán)形撕囊(posterior continuous curvilinear capsulorhexis,PCCC)+前段玻璃體切割(anterior vitrectomy,AV)術(shù),2歲后行二期IOL植入術(shù);2~4歲兒童行白內(nèi)障超聲乳化摘出+PCCC+AV+IOL植入術(shù);4~7歲兒童行白內(nèi)障超聲乳化摘出+PCCC+IOL植入術(shù);7歲兒童行白內(nèi)障超聲乳化摘出+IOL植入術(shù)。結(jié)果共476例(740眼)納入本研究,年齡(33.59±37.14)個月,其中0~6個月166例(282眼,38.11%)。一期植入IOL 260眼,年齡(5.59±2.43)歲;二期植入IOL 480眼,年齡(4.06±2.12)歲。IOL植入術(shù)后殘留屈光度(等效球鏡度數(shù)):2~4歲為(3.53±0.56)D,4~6歲為(2.36±0.32)D,6~8歲為(1.65±0.52)D,8~13歲為(-0.25±0.32)D。術(shù)后并發(fā)癥326眼,其中虹膜粘連115眼(35.28%),位于視軸上的后發(fā)性白內(nèi)障97眼(29.75%),術(shù)后青光眼54眼(16.56%)。末次隨訪時單眼白內(nèi)障術(shù)后最佳矯正視力為(0.56±0.41)LogMAR,雙眼白內(nèi)障術(shù)后最佳矯正視力為(0.42±0.27)LogMAR,兩組相比差異有統(tǒng)計學(xué)意義(P=0.04)。結(jié)論兒童先天性白內(nèi)障約三分之一在6個月內(nèi)進(jìn)行白內(nèi)障超聲乳化摘出術(shù)。雙眼先天性白內(nèi)障患兒視力預(yù)后優(yōu)于單眼先天性白內(nèi)障患兒。兒童先天性白內(nèi)障患兒術(shù)后并發(fā)癥較成人多,需謹(jǐn)慎處理。
