聽力正常青年人寬頻聲導(dǎo)抗研究

【摘要】:目的:獲得聽力正常青年人寬頻聲能吸收率的標(biāo)準(zhǔn)值,并對(duì)寬頻聲導(dǎo)抗測試中的相關(guān)影響因素進(jìn)行初步研究。方法:在首都醫(yī)科大學(xué)附屬北京同仁醫(yī)院臨床聽力學(xué)中心收集86例聽力正常青年人(男女各43例),平均年齡(22.0±1.5)歲。純音測聽在250~8 000Hz范圍各倍頻程內(nèi),氣導(dǎo)聽閾≤25dB HL;226 Hz聲導(dǎo)抗測試鼓室圖均為A型。對(duì)所有受試者行寬頻聲導(dǎo)抗測試,寬頻聲導(dǎo)抗的測量是在226~8 000Hz之間,共107個(gè)倍頻程點(diǎn)進(jìn)行。結(jié)果:共對(duì)172耳的測試結(jié)果進(jìn)行統(tǒng)計(jì)分析。①在250~8 000Hz范圍內(nèi),聲能吸收率測試值在低頻范圍內(nèi)逐漸增大,在1 000~3 000Hz范圍內(nèi)達(dá)到峰值;后隨著頻率增加,聲能吸收率測試值逐漸緩慢減小。②聲能吸收率在4 200~4 900Hz區(qū)間內(nèi)女性受試者測試值明顯高于男性受試者。③性別與226 Hz鼓室圖聲順值、峰壓、寬度之間無顯著相關(guān)性(P=0.319、0.831、0.786);性別與耳道容積之間有明顯的相關(guān)性(P0.01)。④性別與共振頻率之間無顯著相關(guān)性(P0.05)。⑤身高體重與耳道容積有明顯的相關(guān)性(P0.05);耳道容積與共振頻率間同時(shí)具有顯著關(guān)系(P0.05)。結(jié)論:聽力正常青年人寬頻聲能吸收率在低頻時(shí)吸收率較低,迅速增長;中高頻聲能吸收率值較高。寬頻聲能吸收率在不同性別間具有差異性,身高、體重對(duì)寬頻聲導(dǎo)抗的測試結(jié)果存在一定的影響,在臨床應(yīng)用中應(yīng)分別選擇上述參數(shù)的正常值,可增加中耳疾病診斷的特異度和靈敏度。
[Abstract]:Objective: to obtain the standard value of broadband acoustic energy absorption rate in young people with normal hearing. Methods: 86 young people (43 male and 43 male) with normal hearing were collected at Beijing Tongren Hospital affiliated to Capital Medical University. The mean age was 22. 0 鹵1. 5 years. In the range of 250 ~ 8, 000 Hz, the air conduction hearing threshold was less than 25 dB HL. The tympanograms of 226 Hz acoustic conductance test were all type A. all subjects were tested with wide frequency acoustic impedance. The wide band acoustic impedance was measured in the range of 2226 ~ 8 000 Hz. A total of 107 octave points were performed. Results: the results of 172 ears were statistically analyzed. 1 in the range of 250 ~ 8 000 Hz, the acoustic energy absorptivity was gradually increased in the low frequency range. The peak value was reached in the range of 1 000 ~ 3 000 Hz; Then the frequency increases. The sound energy absorption rate of female subjects was significantly higher than that of male subjects in the range of 4 200 ~ 4 900 Hz. Hz tympanogram sound sequence. There was no significant correlation between peak pressure and width. There was no significant correlation between sex and auditory meatus volume. There was no significant correlation between sex and resonance frequency. There was a significant correlation between height, weight and auditory meatus volume (P 0.05). P0.05; There was a significant correlation between auditory meatus volume and resonance frequency (P 0.05). Conclusion: the absorption rate of broadband sound energy in young people with normal hearing is lower and increases rapidly at low frequency. The absorptivity of broadband acoustic energy is higher than that of middle and high frequency. The absorption rate of broadband acoustic energy is different among different genders. Height and weight have some influence on the test results of broadband acoustic conductance. The normal values of the above parameters should be selected in clinical application to increase the specificity and sensitivity of the diagnosis of middle ear diseases.
