【摘要】：目的:對耳內鏡及耳顯微鏡下鼓膜成形術的術中、術后相關指標進行回顧性分析,比較2種不同手術方式的差異性。方法:回顧性分析喀什地區(qū)第二人民醫(yī)院耳鼻咽喉頭頸外科2014-06-2015-08期間70例(70耳)行鼓膜成形術的慢性化膿性中耳炎患者的臨床資料,根據(jù)手術方式分為耳內鏡下鼓膜成形術30例(A組),耳顯微鏡下鼓膜成形術40例(B組)。比較2組的手術時間、術中出血量及術后的干耳時間、切口愈合瘢痕增生情況、鼓膜穿孔率、聽力改善率,利用SPSS 19.0統(tǒng)計軟件分析2組之間的差異性。結果:手術時間:A組為(35.23±6.38)min,B組為(42.60±7.97)min,差異有統(tǒng)計學意義(P0.05)。術中出血量:A組為(7.33±2.11)ml,B組為(17.93±3.84)ml,差異有統(tǒng)計學意義(P0.05)。術后干耳時間超過1個月:A組為40%,B組為75%,差異有統(tǒng)計學意義(P0.05)。術后切口愈合瘢痕增生情況:A組為0%,B組為5%,差異無統(tǒng)計學意義(P0.05)。術后鼓膜穿孔率:A組為3.33%,B組為7.50%,差異無統(tǒng)計學意義(P0.05)。術后聽力改善率:A組為76.67%,B組為80.00%,差異無統(tǒng)計學意義(P0.05)。結論:耳內鏡下鼓膜成形術是一種微創(chuàng)外科技術,與傳統(tǒng)耳顯微鏡下手術相比,術后可以獲得相同的療效,但具有手術時間短、術中出血量少、術后干耳時間短、恢復快、切口美觀等優(yōu)點,值得臨床推廣應用。
[Abstract]:Objective: to analyze retrospectively the operative and postoperative indexes of tympanoplasty under ear endoscope and otoscope. Methods: a retrospective analysis of 70 cases (70 ears) of otolaryngology and head and neck surgery in the second people's Hospital of Kashgar region from 2014-06-2015-08 was made. Clinical data of patients with chronic suppurative otitis media undergoing tympanoplasty. According to the operation mode, 30 cases were divided into two groups: group A (n = 30) and group B (n = 40). The operative time, blood loss during operation and dry ear time after operation were compared between the two groups. Wound healing scar proliferation, tympanic membrane perforation rate, hearing improvement rate. SPSS 19.0 software was used to analyze the difference between the two groups. Results: the operation time of group A was 35.23 鹵6.38 minutes. Group B was 42.60 鹵7.97 min, the difference was statistically significant (P 0.05), and the intraoperative bleeding volume of group A was 7.33 鹵2.11 ml. Group B was 17.93 鹵3.84 ml, the difference was statistically significant (P 0.05). The dry ear time was more than 1 month after operation in group B and group B was 75%. The difference was statistically significant (P 0.05). The rate of scar proliferation in group 0: a was 0 and B was 5%. The rate of perforation of tympanic membrane was 3.33% and 7.50% in group B after operation. There was no significant difference (P 0.05). The improvement rate of hearing in group A was 76.6767 and 80.00% in group B. Conclusion: endoscope tympanoplasty is a minimally invasive surgical technique, which can obtain the same curative effect compared with traditional ear microscope surgery. But it has the advantages of short operation time, less intraoperative bleeding, short dry ear time after operation, quick recovery, beautiful incision and so on.
