【摘要】：[目的]用Meta分析的方法探討肥胖對初次人工全髖關(guān)節(jié)置換術(shù)(total hip arthroplasty,THA)臨床療效的影響。[方法]計算機(jī)檢索數(shù)據(jù)庫Pub Med、EBSCO、The Cochrane Library、Wan Fang、CNKI中關(guān)于肥胖對全髖關(guān)節(jié)置換術(shù)臨床療效的文獻(xiàn),檢索時限為2016年8月1日之前。按納入、排除標(biāo)準(zhǔn)進(jìn)行前瞻性病例對照研究篩查,采用Newcastle Ottawa Scale(NOS)文獻(xiàn)質(zhì)量評價量表對文獻(xiàn)進(jìn)行質(zhì)量評估,采用Rev Man 5.2軟件進(jìn)行Meta分析。[結(jié)果]納入18篇文獻(xiàn),10 047例患者,肥胖組2 985例,非肥胖組7 062例。Meta分析結(jié)果顯示:與非肥胖組相比,肥胖組可以顯著增加THA手術(shù)時間(WMD=10.82,95%CI[4.83,16.82],P=0.000 4)、術(shù)后脫位率(RR=2.08,95%CI[1.55,2.79],P0.000 01)、感染率(RR=3.68,95%CI[2.18,6.22],P0.000 01)、失血量(WMD=169.14,95%CI[73.24,265.05],P=0.000 5),顯著降低Harris Hip Score(HHS)評分(WMD=-3.40,95%CI[-5.41,-1.40],P=0.000 9);但兩組住院時間(WMD=-0.19,95%CI[-0.38,-0.00],P=0.05)差異無統(tǒng)計學(xué)意義。[結(jié)論]肥胖增加人工全髖關(guān)節(jié)置換手術(shù)時間、失血量及術(shù)后脫位、感染并發(fā)癥發(fā)生率,影響患者術(shù)后髖關(guān)節(jié)功能恢復(fù)。肥胖對初次人工全髖關(guān)節(jié)置換術(shù)臨床療效產(chǎn)生負(fù)面影響。
[Abstract]:Objective to investigate the effect of obesity on the clinical efficacy of primary total hip arthroplasty (total hip arthroplasty,THA) by Meta analysis. [methods] the literature on the clinical effect of obesity on total hip arthroplasty in Pub Med,EBSCO,The Cochrane Library,Wan Fang,CNKI was searched by computer for a period of time before August 1, 2016. According to the inclusion and exclusion criteria, the prospective case-control study was screened, the quality of the literature was evaluated by Newcastle Ottawa Scale (NOS) literature quality evaluation scale, and the Meta analysis was carried out by Rev Man 5.2 software.
