【摘要】：目的分析脈沖沖洗術對急性骨與軟組織感染患者炎性介質因子表達的影響及臨床意義。方法選取2011年5月-2016年5月218例骨與軟組織感染患者為研究對象,按照清除創(chuàng)面感染方式的不同,分為傳統(tǒng)組107例,以凡士林紗布覆蓋的方式進行抗感染處理;研究組111例,以脈沖沖洗術進行清創(chuàng)處理,比較兩組患者炎性應激因子超敏C-反應蛋白(hs-CRP)、腫瘤細胞因子-α(TNF-α)、干擾素-γ(IFN-γ)、血沉(ESR)與其他炎性因子白介素-1β(IL-1β)、白介素-6(IL-6)、白介素-8(IL-8)、白介素-10(IL-10)表達的變化及創(chuàng)面正常愈合效果的比較。結果研究組炎性應激指標hs-CRP(2.31±0.56)pg/ml、IFN-γ(22.57±3.76)pg/ml、ESR(13.67±3.82)mm/h表達比傳統(tǒng)組下降明顯,差異有統(tǒng)計學意義(P0.05),TNF-α(38.57±2.76)pg/ml表達比傳統(tǒng)組顯著上升,差異有統(tǒng)計學意義(P0.05);研究組其他炎性因子IL-1β(3.21±0.83)pg/ml、IL-6(78.82±12.84)pg/ml、IL-8(81.46±13.76)pg/ml、IL-10(4.67±2.92)pg/ml表達均較傳統(tǒng)組顯著下降,差異有統(tǒng)計學意義(P0.05)。結論脈沖沖洗術在急性骨與軟組織感染中具有良好的抗菌消炎作用,可明顯改善患者炎性應激因子與其他炎性因子的表達水平,是一種操作性較高的、對治療感染有積極意義的臨床術式。
[Abstract]:Objective to analyze the effect and clinical significance of pulse washing on the expression of inflammatory mediators in patients with acute bone and soft tissue infection. Methods 218 patients with bone and soft tissue infection were selected from May 2011 to May 2016. According to the different ways of clearing the wound, 107 patients in the traditional group were treated with vaseline gauze. The inflammatory stress factor hypersensitive C-reactive protein (hs-CRP), tumor cytokine-偽 (TNF- 偽) and interferon-緯 (IFN- 緯) were compared between the two groups. The changes of expression of IL-1 尾, IL-6, IL-8, IL-10 in erythrocyte sedimentation rate (ESR) and other inflammatory factors (IL-1 尾, IL-6, IL-8, IL-10) and the effect of normal wound healing were compared. Results the expression of inflammatory stress index hs-CRP (2.31 鹵0.56) pg/ml,IFN- 緯 (22.57 鹵3.76) pg/ml,ESR (13.67 鹵3.82) mm/h in the study group was significantly lower than that in the traditional group (P0.05). The expression of TNF- 偽 (38.57 鹵2.76) pg/ml was significantly higher than that of the traditional group (P0.05). The expression of other inflammatory factors, IL-1 尾 (3.21 鹵0.83) pg/ml,IL-6 (78.82 鹵12.84) pg/ml,IL-8, (81.46 鹵13.76) pg/ml,IL-10, (4.67 鹵2.92) pg/ml in the study group was significantly lower than that in the traditional group. The difference was statistically significant (P0.05). Conclusion Pulsed washing has a good antibacterial and anti-inflammatory effect in acute bone and soft tissue infection and can significantly improve the expression level of inflammatory stress factors and other inflammatory factors in patients with acute bone and soft tissue infection. A clinical procedure that has a positive effect on the treatment of infection.
