肩峰成形對肩袖損傷縫線橋修復術后功能的影響

【摘要】：目的:評價關節(jié)鏡下肩峰成形術在肩袖損傷修復中的臨床療效。方法:2012年5至2014年5月接受關節(jié)鏡下肩袖損傷縫線橋固定的患者65例,男42例,女23例,隨機分為兩組,一組術中同時行關節(jié)鏡下肩峰成形手術(實驗組),另一組術中行關節(jié)鏡下肩峰下清理,不做骨質的切除(對照組)。兩組患者年齡、性別、側別的差異均無統(tǒng)計學意義。記錄兩組手術時間,比較術后3個月、12個月時肩關節(jié)活動度、美國加州大學洛杉磯分校評分系統(tǒng)(ULCA)、美國肩肘外科醫(yī)師評分系統(tǒng)(ASES)、視覺模擬評分(VAS)和Constant評分變化。結果:實驗組與對照組在手術時間上兩組差異有統(tǒng)計學意義(t=-18.5,P0.05);實驗組與對照組術后3個月、12個月肩關節(jié)活動范圍、肩關節(jié)功能評分方面兩組差異無統(tǒng)計學意義(P0.05)。結論:關節(jié)鏡下肩袖損傷縫線橋固定術中進行肩峰成形手術在術后肩關節(jié)活動度、肩關節(jié)評分的改善方面與對照組無顯著差異,但手術時間更長。
[Abstract]:Objective: to evaluate the clinical effect of arthroscopic acromioplasty in the repair of rotator cuff injury. Methods: from May 2012 to May 2014, 65 patients (42 males and 23 females) with arthroscopic suture bridge fixation of rotator cuff injury were randomly divided into two groups: one group underwent arthroscopic acromioplasty simultaneously (experimental group). Another group underwent arthroscopic subacromial debridement without bone resection (control group). There was no significant difference in age, sex and side difference between the two groups. The operation time of the two groups was recorded, and the range of shoulder motion at 3 and 12 months postoperatively was compared. The UCLA scoring system (ULCA), the American shoulder and elbow surgeon scale system, (ASES), Visual analogue score: (VAS) and Constant. Results: there was significant difference in the operation time between the experimental group and the control group (t = -18.5). There was no significant difference between the experimental group and the control group in the range of shoulder motion and the shoulder function score 3 months and 12 months after operation (P0.05). Conclusion: there is no significant difference in the range of shoulder motion and the improvement of shoulder joint score between arthroscope and the control group, but the operative time is longer.
