【摘要】：目的探討靜脈聯合關節(jié)腔內注射氨甲環(huán)酸對全髖關節(jié)置換圍手術期出血及深靜脈血栓形成的影響。方法選擇行單側全髖關節(jié)置換的102例患者進行研究。采用隨機數字表法將其分為4組:A組26例,靜脈聯合關節(jié)腔內注射氨甲環(huán)酸;B組26例,靜脈注射氨甲環(huán)酸;C組25例,關節(jié)腔內注射氨甲環(huán)酸;D組25例,未使用氨甲環(huán)酸。比較4組術中失血量、術后引流量、術后總失血量、血紅蛋白含量、輸血量及深靜脈血栓發(fā)生率。結果術后引流量、術后總失血量、血紅蛋白含量、輸血量,A組優(yōu)于B、C組,B組優(yōu)于C、D組,C組優(yōu)于D組,差異均有統(tǒng)計學意義(P0.05)。4組患者術中失血量及深靜脈血栓發(fā)生率差異無統(tǒng)計學意義(P0.05)。結論靜脈聯合關節(jié)腔內注射氨甲環(huán)酸能明顯降低全髖關節(jié)置換圍手術期失血量,并且不增加深靜脈血栓形成發(fā)生的風險,較單獨使用氨甲環(huán)酸效果更好。
[Abstract]:Objective to investigate the effect of intraarticular injection of ammonia cycline on perioperative hemorrhage and deep venous thrombosis in total hip arthroplasty (THR). Methods 102 patients with unilateral total hip replacement were studied. They were randomly divided into four groups: group A (n = 26), group B (n = 26), group C (n = 25), intraarticular injection of aminoformic acid; Group D, 25 cases, did not use carbonic acid. The blood loss, postoperative drainage, total blood loss, hemoglobin content, blood transfusion volume and the incidence of deep venous thrombosis were compared among the four groups. Results postoperative drainage, total blood loss, hemoglobin content and blood transfusion volume in group A were better than those in group B, group B and group C were superior to group C and group D, respectively. The difference was statistically significant (P0.05). There was no significant difference in intraoperative blood loss and the incidence of deep venous thrombosis in the 4 groups (P0.05). Conclusion intravenous and intraarticular injection of aminocylic acid can significantly reduce blood loss in perioperative period of total hip replacement and does not increase the risk of deep venous thrombosis.
