【摘要】：背景:目前國內(nèi)行疝補(bǔ)片手術(shù),一般根據(jù)經(jīng)驗(yàn)裁剪補(bǔ)片大小,缺乏可靠的依據(jù)。目的:通過Rutkow術(shù)中測量中國籍成年男性人群腹股溝后壁大小,根據(jù)測量結(jié)果進(jìn)行補(bǔ)片裁剪,對治療效果進(jìn)行評價。方法:精細(xì)測量180例行無張力疝修補(bǔ)術(shù)的中國籍成年男性患者的腹股溝后壁大小,計算出適合中國成年男性腹股溝組織解剖結(jié)構(gòu)的補(bǔ)片尺寸數(shù)據(jù),并將180例患者隨機(jī)分為2組,每組90例,試驗(yàn)組測量腹股溝后壁大小后進(jìn)行補(bǔ)片裁剪的Rutkow術(shù),對照組根據(jù)經(jīng)驗(yàn)進(jìn)行補(bǔ)片裁剪的Rutkow術(shù),比較兩組患者各項(xiàng)臨床指標(biāo)以及并發(fā)癥發(fā)生率和復(fù)發(fā)率。結(jié)果與結(jié)論:1恥骨結(jié)節(jié)與腹內(nèi)斜肌在腹股溝韌帶起點(diǎn)之間距離為(6.03±0.58)cm;弓狀下緣的最高點(diǎn)至腹股溝韌帶的垂直距離為(3.02±0.32)cm,相應(yīng)補(bǔ)片尺寸長度應(yīng)當(dāng)大于(13.04±0.74)cm,寬度應(yīng)當(dāng)大于(5.88±0.52)cm;2試驗(yàn)組下床活動時間、進(jìn)食時間、住院時間均小于對照組(P0.05);兩組手術(shù)時間、恢復(fù)正�；顒訒r間比較,差異無顯著性意義(P0.05);3試驗(yàn)組術(shù)后并發(fā)癥發(fā)生率及復(fù)發(fā)率明顯低于對照組(P0.01);4結(jié)果表明,測量腹股溝后壁大小后進(jìn)行補(bǔ)片裁剪的Rutkow術(shù)用于腹股溝疝修補(bǔ),效果明顯,并發(fā)癥和復(fù)發(fā)率低。
[Abstract]:Background: at present, domestic herniation patch surgery, generally based on experience to cut the size of the patch, lack of reliable basis. Objective: to measure the posterior wall size of inguinal groin in Chinese adult male population by Rutkow, and to evaluate the therapeutic effect. Methods: the size of inguinal posterior wall of 180 Chinese adult male patients undergoing tension-free hernioplasty was measured, and the size data of the patch suitable for the anatomical structure of inguinal tissue in Chinese adult males were calculated. 180 patients were randomly divided into two groups. 90 patients in each group were treated with Rutkow after the size of the posterior wall of groin were measured, and the patients in the control group were treated with Rutkow according to their experience. The clinical indexes, the incidence of complications and the recurrence rate were compared between the two groups. Results and conclusion: 1 the distance between pubic tubercle and internal oblique muscle at the beginning of inguinal ligament is (6.03 鹵0.58) cm;. The vertical distance between the highest point of the arch lower edge and the inguinal ligament is (3.02 鹵0.32) cm,. The corresponding patch size length should be greater than (13.04 鹵0.74) cm, width should be greater than (5.88 鹵0.52) cm;. (2) the time of getting out of bed, eating time and hospitalization time in the test group were all smaller than those in the control group (P0.05), the operation time and the time of returning to normal activity were not significantly different between the two groups (P0.05). (3) the incidence and recurrence rate of postoperative complications in the trial group were significantly lower than those in the control group (P0.01). 4 the results showed that the effect of Rutkow after the measurement of the size of the posterior wall of the groin for inguinal hernia repair was obvious, and the complication and recurrence rate was low.
