【摘要】：目的:觀察自體骨髓間充質干細胞植入聯(lián)合髓芯減壓植骨支撐術治療早中期股骨頭壞死的臨床效果。方法:選取2016年3月～2017年3月在我院就診的80例早中期股骨頭壞死患者作為研究對象,采用隨機數(shù)字表法分為對照組和觀察組,每組40例。對照組采用髓芯減壓植骨術治療,觀察組采用自體骨髓間充質干細胞植入聯(lián)合髓芯減壓植骨支撐術治療。比較兩組的治療效果、疼痛程度和Harris關節(jié)功能評分。結果:觀察組的治療優(yōu)良率明顯高于對照組,差異有統(tǒng)計學意義,P0.05;治療前,兩組的VAS評分和Harris評分相比較,差異均無統(tǒng)計學意義,P0.05;治療后,兩組的VAS評分均較治療前明顯降低,兩組的Harris評分均較治療前明顯升高,且觀察組的疼痛程度降低幅度和關節(jié)功能改善幅度均優(yōu)于對照組,差異均有統(tǒng)計學意義,P0.05。結論:自體骨髓間充質干細胞植入聯(lián)合髓芯減壓植骨支撐術治療早中期股骨頭壞死臨床效果顯著,可有效改善患者的關節(jié)功能,減輕疼痛程度,總體療效相對較高。
[Abstract]:Objective: to observe the clinical effect of autologous bone marrow mesenchymal stem cells implantation combined with core decompression and bone grafting in the treatment of early and middle stage femoral head necrosis. Methods: 80 patients with osteonecrosis of femoral head from March 2016 to March 2017 in our hospital were selected and randomly divided into control group (n = 40) and observation group (n = 40). The control group was treated with bone marrow core decompression and bone grafting, while the observation group was treated with autologous bone marrow mesenchymal stem cells implantation combined with bone marrow core decompression and bone grafting. The therapeutic effect, pain degree and Harris joint function score were compared between the two groups. Results: the excellent and good rate of treatment in the observation group was significantly higher than that in the control group, the difference was statistically significant (P 0.05). Before treatment, there was no significant difference in VAS score and Harris score between the two groups (P 0.05). After treatment, the VAS score of the two groups was significantly lower than that of the control group, and the Harris score of the two groups was significantly higher than that of the control group, and the degree of pain reduction and the improvement of joint function in the observation group were better than those in the control group, and the difference was statistically significant. P0.05. Conclusion: the clinical effect of autologous bone marrow mesenchymal stem cells implantation combined with core decompression and bone grafting in the treatment of femoral head necrosis in the early and middle period is remarkable. It can effectively improve the joint function and alleviate the degree of pain. The overall curative effect is relatively high.
