上肢功率車在脊髓損傷截癱患者心肺運(yùn)動(dòng)試驗(yàn)中的應(yīng)用研究

【摘要】:目的:觀察上肢功率車在脊髓損傷(spinal cord injury,SCI)截癱患者心肺運(yùn)動(dòng)試驗(yàn)中應(yīng)用的可行性并分析SCI截癱患者的心肺功能狀態(tài),探討SCI截癱患者有氧運(yùn)動(dòng)適合的評(píng)價(jià)指標(biāo)。方法:招募20例SCI截癱患者(試驗(yàn)組)及20名健康人(對(duì)照組),所有受試者在K4b2氣體分析儀的監(jiān)測(cè)下,采用智能主被動(dòng)上肢功率車進(jìn)行心肺運(yùn)動(dòng)試驗(yàn)。受試者進(jìn)入實(shí)驗(yàn)室,休息10min后開(kāi)始上肢功率車運(yùn)動(dòng),轉(zhuǎn)速主動(dòng)控制在50—60r/min。起始功率為0W,并以每2min增加10W的幅度遞增,直至測(cè)試者出現(xiàn)限制性癥狀終止試驗(yàn),監(jiān)測(cè)整個(gè)運(yùn)動(dòng)過(guò)程的心肺指標(biāo)、主觀勞累程度、心率、血壓等。結(jié)果:測(cè)試過(guò)程中兩組受試者心率、收縮壓及舒張壓在時(shí)間效應(yīng)上存在顯著差異(P0.001);試驗(yàn)組與對(duì)照組比較發(fā)現(xiàn),峰值攝氧量(19.38±2.56ml/min/kg,26.49±3.45ml/min/kg)、最大心率(143.35±20.01bmp,170.95±11.93bmp)、無(wú)氧閾時(shí)心率(102.35±12.16bmp,120.25±10.07bmp)、呼吸交換率的最大值(1.15±0.04,1.22±0.05)、運(yùn)動(dòng)時(shí)間(12.55±0.61min,13.47±1.13min)差異有顯著性意義(P0.01);試驗(yàn)組無(wú)氧閾時(shí)所對(duì)應(yīng)的主觀勞累程度與對(duì)照組比較,差異無(wú)顯著性意義(P0.05)。試驗(yàn)組獲得的最大心率約為預(yù)期值的67%,對(duì)照組獲得的最大心率約為預(yù)期值得85%。結(jié)論:對(duì)SCI截癱患者采用上肢功率車運(yùn)動(dòng)進(jìn)行心肺運(yùn)動(dòng)試驗(yàn)具有一定的可行性,SCI截癱患者的心肺功能均有不同程度下降;主觀勞累程度可作為此類患者有氧運(yùn)動(dòng)強(qiáng)度的適宜參數(shù)。
[Abstract]:Objective: to observe the feasibility of the application of upper limb power car in cardiopulmonary motor test in paraplegic patients with spinal cord injury (spinal cord injury,SCI), and to analyze the cardiopulmonary function of SCI paraplegia patients, and to explore the evaluation index of aerobic exercise for SCI paraplegia patients. Methods: twenty patients with SCI paraplegia (experimental group) and 20 healthy persons (control group) were recruited. All subjects were monitored by K4b2 gas analyzer and the cardiopulmonary exercise test was carried out with intelligent active and passive upper limb power vehicle. The subjects entered the laboratory and rested for 10min, then began to move the upper limb power car, and the rotational speed was controlled at 50 ~ 60 r / min. The initial power was 0 W and increased by 10 W per 2min until the test was terminated with restrictive symptoms and the cardiopulmonary indexes subjective tiredness heart rate and blood pressure were monitored during exercise. Results: there were significant differences in the time effect of heart rate, systolic blood pressure and diastolic blood pressure between the two groups (P0.001). The maximal oxygen uptake (19.38 鹵2.56ml / kg 路min / kg 26.49 鹵3.45ml/min/kg), the maximum heart rate (143.35 鹵20.01bmpg) 170.95 鹵11.93bmp, the heart rate (102.35 鹵12.16bmpg, 120.25 鹵10.07bmp), the maximum respiratory exchange rate (1.15 鹵0.041.22 鹵0.05), the exercise time (12.55 鹵0.61min, 13.47 鹵1.13min) were significantly different (P0.01), and there was no significant difference in the subjective tiredness between the experimental group and the control group (P0.05). The maximum heart rate obtained in the trial group was about 67% of the expected value, and the maximum heart rate in the control group was about 85% of the expected value. Conclusion: it is feasible to conduct cardiopulmonary exercise test in patients with SCI paraplegia by upper limb power vehicle exercise, and the cardiopulmonary function of SCI paraplegia patients is decreased to some extent. Subjective tiredness can be used as an appropriate parameter for aerobic exercise intensity of such patients.
