【摘要】：目的對比分析自固定補片(Pro Grip)與超普網(wǎng)塞補片(UPP)在無張力疝修補術(shù)中的臨床應用效果。方法收集黔西南州人民醫(yī)院2015年1至12月確診為腹股溝斜疝或直疝的92例患者,按入院的順序編號,奇數(shù)為試驗組(47例),偶數(shù)為對照組(45例),試驗組采用自固定補片(Pro Grip),而對照組采用超普網(wǎng)塞補片(UPP),均在全麻或腰硬聯(lián)合麻醉下行無張力疝修補術(shù)。觀察2組患者手術(shù)時間、住院時間、住院費用、術(shù)后并發(fā)癥(切口感染、陰囊積液、復發(fā))、術(shù)后異物感、術(shù)后疼痛等臨床指標的變化。疼痛評分用視覺模擬評分表記錄。隨訪時間15個月。結(jié)果 2組患者在年齡、性別、疝的類型、合并癥等臨床資料方面比較,差異無統(tǒng)計學意義(P0.05)。2組平均手術(shù)時間、住院時間及術(shù)后第3天、1、3、9、15個月術(shù)后疼痛程度方面自固定補片組優(yōu)于超普網(wǎng)塞補片組,差異有統(tǒng)計學意義(P0.05)。而自固定補片組住院費用略高,與超普網(wǎng)塞補片組比較差異有統(tǒng)計學意義(P0.05)。術(shù)后第1、3、6、9、15個月異物感發(fā)生率及術(shù)后并發(fā)癥(切口感染、陰囊積液、復發(fā))發(fā)生率等方面比較,2組差異無統(tǒng)計學意義(P0.05)。結(jié)論自固定補片組住院總費用略高于超普網(wǎng)塞補片組,術(shù)后異物感發(fā)生率、術(shù)后并發(fā)癥(切口感染、陰囊積液、復發(fā))發(fā)生率無明顯差別,而用自固定補片行疝修補術(shù)具有手術(shù)時間、住院時間短,術(shù)后疼痛輕等優(yōu)點。因此,自固定補片有望在疝修補術(shù)中得到普及。
[Abstract]:Objective to compare and analyze the clinical effect of (Pro Grip) and (UPP) in tension-free hernia repair. Methods 92 patients diagnosed as indirect inguinal hernia or direct hernia from January to December 2015 in Qianxinan people's Hospital were collected and numbered according to the order of admission. The odd number was the experimental group (47 cases) and the even number was the control group (45 cases). The experimental group was treated with self-fixed patch (Pro Grip), while the control group was treated with supernormal mesh plug patch (UPP), under general anesthesia or combined spinal-epidural anesthesia with tension-free herniorrhaphy. The changes of operation time, hospitalization cost, postoperative complications (incision infection, scrotum effusion, recurrence), postoperative foreign body sensation and postoperative pain were observed. Pain scores were recorded using visual analogue scale. The follow-up period was 15 months. Results there was no significant difference in age, sex, type of hernia and complications between the two groups (P0.05). The length of hospital stay and postoperative 3 days, 1, 3, 9, 15 months of postoperative pain in the self-fixed patch group was better than that in the ultra-plain mesh patch group, the difference was statistically significant (P0.05). The hospitalization cost of the self-fixed patch group was slightly higher than that of the supernormal mesh plug group (P0.05). There was no significant difference in the incidence of foreign body sensation and postoperative complications (incision infection scrotum effusion recurrence) between the two groups (P0.05). Conclusion the total cost of hospitalization in the self-fixed patch group was slightly higher than that in the supernormal mesh patch group. There was no significant difference in the incidence of foreign body sensation and postoperative complications (incision infection, scrotal effusion, recurrence). The herniorrhaphy with self-fixed patch has the advantages of short time of operation, short hospital stay and mild postoperative pain. Therefore, self-fixed patch is expected to be widely used in hernia repair.
