磁共振動態(tài)增強及波譜成像對骨肉瘤的鑒別診斷價值

【摘要】:目的探討磁共振動態(tài)增強(DCE)曲線類型及氫質(zhì)子波譜成像(~1H-MRS)對原發(fā)性骨肉瘤的應用價值。資料與方法前瞻性納入41例符合研究標準的骨肉瘤患者,分別行DCE及~1H-MRS掃描,根據(jù)其病理結(jié)果分為高度和低度惡性骨肉瘤,利用分析軟件得到時間-信號強度曲線(TIC)并進行分類;對其中18例~1H-MRS譜線穩(wěn)定的骨肉瘤患者進行分析并得到Cho及Cho/Cr值,分別進行統(tǒng)計分析。結(jié)果高度惡性骨肉瘤中TIC Ⅰ型、Ⅱ型曲線分別有20例、10例,低度惡性骨肉瘤TIC Ⅰ型、Ⅱ型、Ⅲ型曲線分別有2例、5例、4例,高、低度惡性骨肉瘤之間TIC類型分布差異有統(tǒng)計學意義(χ~2=10.876,P0.05)。高、低度惡性骨肉瘤的Cho/Cr值差異有統(tǒng)計學意義(Z=-2.445,P0.05)。以Cho/Cr作為診斷指標,最佳診斷界值為2.3。結(jié)論 TIC類型、Cho/Cr值對于評價骨肉瘤的惡性程度有一定的參考價值。
[Abstract]:Objective to investigate the type of dynamic enhanced (DCE) curve and the value of hydrogen proton spectrum imaging (~ 1H-MRS) in primary osteosarcoma. Materials and methods 41 patients with osteosarcoma who met the study criteria were included prospectively. DCE and ~ 1H-MRS scans were performed respectively. According to the pathological results, they were divided into high and low grade malignant osteosarcoma.
