【摘要】：[目的]探討MRI影像分型對胸腰椎布魯桿菌病性脊椎炎手術治療選擇及療效分析。[方法]將2005年1月~2015年2月167例布魯桿菌病性脊椎炎患者的MRI影像學資料,依據(jù)MRI分型行微創(chuàng)術與切開手術治療;通過影像分析一致性檢驗、影像學評分、臨床療效評價進行性歸納總結。采用SPSS 15.0統(tǒng)計軟件包進行分析。[結果]167例治療前行MRI檢查,6個月126例復查;12個月113例復查,影像分析一致性檢驗Kappa值均0.75。依據(jù)術前MRI影像分型其治療結果:選擇微創(chuàng)14例(A組);153例選擇病灶清除植骨術,56例未行內固定(B組),97例采用內固定(C組);各組12個月治愈率與前一個時間點比較差異有統(tǒng)計學意義(P0.05);在6、12個月的時間點C組治愈率明顯優(yōu)于A組及B組,且A組優(yōu)于B組,差異均有統(tǒng)計學意義(P0.05)。[結論]布魯桿菌病性脊椎炎MRI影像學分型對微創(chuàng)和手術治療策略的制定具有重要價值及指導意義,內固定治療布魯桿菌病性脊椎炎效果優(yōu)于其他治療方法。
[Abstract]:[objective] to investigate the choice of surgical treatment and the curative effect of MRI imaging classification for thoracic and lumbar spondylitis of brucellosis. [methods] the MRI imaging data of 167 patients with brucellosis spondylitis from January 2005 to February 2015 were treated with minimally invasive surgery and open surgery according to MRI classification. Clinical efficacy evaluation progressive induction and summary. SPSS 15.0 statistical software package was used for analysis. [results] MRI examination was performed in 167 cases before treatment, 126 cases at 6 months and 113 cases at 12 months. The Kappa value of image analysis consistency test was 0.75. According to preoperative MRI imaging classification, the treatment results were as follows: 14 cases (group A), 153 cases of lesion debridement and bone grafting (group B), 97 cases without internal fixation (group C), the cure rate of 12 months in each group was compared with that in the previous time point. The cure rate of group C was significantly better than that of group A and group B at the time point of 6,12 months (P0.05). Group A was better than group B, the difference was statistically significant (P0.05). [conclusion] MRI imaging classification of brucellosis spondylitis has important value and guiding significance in the formulation of minimally invasive and surgical treatment strategy, and the effect of internal fixation on brucellosis spondylitis is superior to other treatment methods.
