體感誘發(fā)電位聯(lián)合運動誘發(fā)電位監(jiān)測在頸動脈內(nèi)膜剝脫術(shù)中的應(yīng)用

【摘要】:目的評估在頸動脈內(nèi)膜剝脫術(shù)中(carotid endarterectomy,CEA)中采用體感誘發(fā)電位(somatosensory evoked potential,SSEP)與運動誘發(fā)電位(motor evoked potential,MEP)聯(lián)合監(jiān)測的方案對于預(yù)防術(shù)中腦缺血發(fā)生的準確性。方法選擇因頸動脈狹窄擇期擬行CEA患者90例,男71例,女19例,年齡18~80歲,ASAⅡ或Ⅲ級。術(shù)中監(jiān)測SSEP和MEP,記錄頸內(nèi)動脈阻斷前、頸動脈阻斷時、阻斷期間及開放后直至術(shù)畢SSEP和MEP波幅和潛伏期。評估術(shù)后5d內(nèi)神經(jīng)功能缺失情況,以發(fā)生神經(jīng)功能缺失作為評判術(shù)中腦缺血發(fā)生的金標準。結(jié)果本研究中14例(15.6%)患者發(fā)生術(shù)后神經(jīng)功能缺失。SSEP預(yù)測腦缺血發(fā)生的靈敏度79%、特異度92%;MEP預(yù)測腦缺血發(fā)生的靈敏度86%、特異度89%、SSEP+MEP聯(lián)合監(jiān)測的靈敏度為79%、特異度99%。結(jié)論在頸動脈內(nèi)膜剝脫術(shù)中,體感誘發(fā)電位預(yù)測腦缺血發(fā)生的特異度高,運動誘發(fā)電位靈敏度高;二者聯(lián)合監(jiān)測可提高監(jiān)測的特異性,彌補單一監(jiān)測的不足。
