【摘要】：目的探討乳腺結(jié)節(jié)性筋膜炎的臨床病理學(xué)特征、診斷及鑒別診斷。方法采用HE染色和免疫組化En Vision法檢測(cè)4例乳腺結(jié)節(jié)性筋膜炎,并進(jìn)行臨床病理回顧性分析。結(jié)果患者均為女性,中位年齡50.5歲,病程短,無(wú)局部外傷史,腫塊平均直徑1.2 cm,3例位于乳腺實(shí)質(zhì),1例緊鄰皮膚。4例病變均見不同程度梭形細(xì)胞增生和紅細(xì)胞外滲及不等量淋巴細(xì)胞浸潤(rùn),其中3例邊界不清,浸潤(rùn)周圍乳腺及脂肪組織。3例行術(shù)中冷凍,2例與常規(guī)診斷完全符合。免疫表型:梭形細(xì)胞表達(dá)vimentin、CD10和SMA,不表達(dá)CK(AE1/AE3)、CK5/6、CK14、CAM5.2、ER和PR。隨訪6~93個(gè)月,4例經(jīng)手術(shù)局部切除后,均未復(fù)發(fā)。結(jié)論乳腺結(jié)節(jié)性筋膜炎臨床較少見,術(shù)中冷凍診斷有一定的難度,易與乳腺纖維瘤病、葉狀腫瘤及低級(jí)別纖維瘤病樣梭形細(xì)胞癌等良、惡性腫瘤混淆,免疫組化標(biāo)記vimentin和SMA對(duì)診斷及鑒別診斷乳腺結(jié)節(jié)性筋膜炎有幫助。
[Abstract]:Objective to investigate the clinicopathological features, diagnosis and differential diagnosis of nodular fasciitis of breast. Methods four cases of nodular fasciitis of mammary gland were detected by HE staining and immunohistochemical en Vision method. Results all the patients were female, the median age was 50.5 years old, the course of disease was short and there was no history of local trauma. The mean diameter of the mass was 1.2 cm in 3 cases. One case was located in the parenchyma of the mammary gland. One case was adjacent to the skin. All the lesions showed various degrees of spindle cell proliferation, exosmosis of red blood cell and infiltration of different amount of lymphocytes. The boundary of 3 cases was not clear. Intraoperative cryopreservation was performed in 2 cases of peri-invasive breast and adipose tissue, which was in good agreement with routine diagnosis. Immunophenotypes: spindle cells expressed vimentinine CD10 and SMA. they did not express CK (AE1 / AE3), CK5 / 6, CK14, CAM5.2, ER and PR. Follow-up of 6 ~ 93 months showed no recurrence in 4 cases after local resection. Conclusion Breast nodular fasciitis is rare in clinic and it is difficult to diagnose by cryopreservation during operation. It is easy to be confused with benign and malignant tumor such as breast fibromatosis, lobular tumor and low-grade fibromatous spindle cell carcinoma. Immunohistochemical staining of vimentin and SMA is helpful in the diagnosis and differential diagnosis of nodular fasciitis of breast.
