【摘要】：目的通過回顧性分析冠狀動脈瘺合并擴張手術(shù)治療的臨床資料,提出對這種復(fù)雜畸形的診療經(jīng)驗。方法回顧性分析2001年5月至2016年10月,共11例先天性冠狀動脈瘺合并冠狀動脈擴張的患者在解放軍總醫(yī)院心血管外科實施手術(shù)治療的臨床資料,根據(jù)不同冠脈解剖特點采取不同的手術(shù)方式,總結(jié)死亡和并發(fā)癥發(fā)生率等臨床特征性指標(biāo),并對患者進行隨訪。結(jié)果 1例患者術(shù)后心梗嚴(yán)重低心排死亡。體外循環(huán)時間(104.8±44.9)min,阻斷時間(71.9±34.1)min,呼吸機輔助呼吸(26.0±20.5)h,術(shù)后出院時間(12.5±7.7)d。術(shù)后2例患者出現(xiàn)室性心律失常;心包填塞1例;心肌梗死2例。其余患者無并發(fā)癥發(fā)生。9例患者經(jīng)電話或再次住院隨訪,1例患者失訪。隨訪患者均無癥狀存活。平均隨訪58.2(10~105)個月。無因心臟事件再次手術(shù)患者。結(jié)論對冠狀動脈瘺合并擴張這種少見畸形患者,冠狀動脈造影及多源螺旋CT三維重建是診斷的重要方法,一經(jīng)確診應(yīng)手術(shù)治療,術(shù)式應(yīng)選擇個體化方案,手術(shù)近遠(yuǎn)期療效肯定。
[Abstract]:Objective to retrospectively analyze the clinical data of coronary artery fistula complicated with dilatation operation and put forward the experience of diagnosis and treatment of this complex malformation. Methods from May 2001 to October 2016, 11 patients with congenital coronary artery fistula complicated with coronary artery dilatation were analyzed retrospectively. According to different anatomical characteristics of coronary artery, different surgical methods were used to summarize the clinical characteristics of death and complications, and follow up the patients. Results one patient died of severe low cardiac output after myocardial infarction. The CPB time was (104.8 鹵44.9) min, the blocking time was (71.9 鹵34.1) min, the ventilator assisted respiration time was (26.0 鹵20.5) h, and the postoperative discharge time was (12.5 鹵7.7) d. Ventricular arrhythmia occurred in 2 cases, pericardial tamponade in 1 case and myocardial infarction in 2 cases. There were no complications in the rest of the patients. 9 cases were followed up by telephone or re-hospitalization, and 1 case was not visited. All patients were asymptomatic and survived. The mean follow-up was 58.2 (10 / 105) months. No reoperation due to cardiac events. Conclusion for patients with coronary artery fistula complicated with dilatation, coronary angiography and three-dimensional reconstruction of multi-source spiral CT are important methods for diagnosis. Once the diagnosis is confirmed, the surgical treatment should be individualized, and the short-term and long-term results of the operation are positive.
