【摘要】：目的·利用經(jīng)食管實時三維超聲定量評估成形術對二尖瓣三維結構的形態(tài)學影響。方法·收集36例二尖瓣脫垂手術組患者瓣膜成形前后及56例對照組患者的三維超聲資料,采集瓣環(huán)及瓣葉三維結構參數(shù)并對比分析。結果·術前手術組橢圓指數(shù)、非平面夾角小于對照組,其余參數(shù)大于對照組,差異有統(tǒng)計學意義。術后瓣環(huán)的前后徑、前外后內(nèi)直徑、橢圓指數(shù)、最小三維周長、二維投影中的面積、最小三維面積及瓣葉的前瓣葉暴露面積、交界直徑小于對照組,瓣環(huán)非平面夾角大于對照組,差異有統(tǒng)計學意義;其余參數(shù)與對照組比較,無明顯差異。后葉人工腱索技術組與瓣葉三角形或矩形切除技術組手術前后各參數(shù)均無明顯差異。結論·成形術可對二尖瓣功能進行滿意糾治,但從形態(tài)學角度觀察,術后瓣環(huán)形態(tài)并沒有恢復至正常,而瓣葉的三維形態(tài)基本得到滿意糾治。人工腱索技術與瓣葉三角形或矩形切除技術對后葉脫垂的形態(tài)學療效相仿。
[Abstract]:Objective to evaluate the morphologic changes of mitral valve by transesophageal real-time three-dimensional ultrasound. Methods three dimensional ultrasound data were collected from 36 patients with mitral valve prolapse before and after valvuloplasty and 56 patients with control group. The three dimensional structural parameters of annulus and lobes were collected and analyzed. Results the elliptic index and non-plane angle of the operation group were lower than those of the control group, and the other parameters were larger than those of the control group, the difference was statistically significant. The anteroposterior diameter, anterior and posterior diameter, elliptic index, minimum three-dimensional circumference, area in two-dimensional projection, minimum area of anterior lobe and the area of anterior lobe exposure were smaller than those of the control group, and the diameter of the anterior lobe was smaller than that of the control group. The nonplanar angle of valve annulus was higher than that of control group, the difference was statistically significant, and the other parameters had no significant difference with the control group. There was no significant difference between the posterior lobe artificial chordae tendineae technique group and the flap triangular or rectangular resection group before and after operation. Conclusion Mitral valvuloplasty can be used to correct the mitral valve function satisfactorily, but from the morphological point of view, the annular shape does not return to normal, and the three-dimensional shape of the valve lobe is basically satisfactorily corrected. The effect of artificial chordae tendineae on posterior lobe prolapse was similar to that of triangular or rectangular flap resection.
