同種帶瓣主動脈去內(nèi)皮處理的基礎(chǔ)研究

【摘要】:目的比較去內(nèi)皮同種帶瓣主動脈與傳統(tǒng)去細胞同種瓣的生物學(xué)特性和免疫原性,探究性能更優(yōu)越的同種瓣預(yù)處理方法。方法獲取家兔帶瓣主動脈根部組織,隨機分為新鮮組(獲取后無特殊處理,作為空白對照組)、去內(nèi)皮細胞組(應(yīng)用膠原酶處理法脫去內(nèi)皮細胞)、去全部細胞組(應(yīng)用傳統(tǒng)方法脫去組織中全部的細胞成分)。應(yīng)用HE、Masson、VG染色法及掃描電鏡進行組織形態(tài)學(xué)觀察,通過對比測定各組組織厚度、抗張強度、斷裂拉伸率及熱皺縮溫度進行生物學(xué)特性評價,通過CD152免疫組織化學(xué)染色觀察及原子火焰吸收法測定移植物鈣含量變化進行免疫原性評價。結(jié)果組織形態(tài)學(xué)觀察結(jié)果提示:去內(nèi)皮和去細胞處理均達到了理想的脫細胞目的。三種組織的厚度、熱皺縮溫度無統(tǒng)計學(xué)差異(P0.05)。與新鮮組相比,去內(nèi)皮細胞組抗張力性能變化不明顯(P0.05),去全部細胞組抗張性能明顯下降(P0.01)。新西蘭兔皮下包埋實驗后HE染色觀察發(fā)現(xiàn):新鮮組炎性反應(yīng)最為劇烈,包埋后有大量炎細胞浸潤;與新鮮組相比,去內(nèi)皮細胞組包埋后炎性反應(yīng)顯著減弱,炎細胞數(shù)目明顯減少,去全部細胞組炎性反應(yīng)最弱,僅組織邊緣有少量炎細胞浸潤。免疫組化觀察發(fā)現(xiàn)去內(nèi)皮細胞組和去全部細胞組CD152表達水平明顯高于對照組(P0.05),而去內(nèi)皮細胞組與去全部細胞組之間未見顯著差異(P0.05)。鈣含量分析結(jié)果提示:去內(nèi)皮細胞組和去全部細胞組包埋后鈣鹽增加量較新鮮組顯著下降(P0.05),去內(nèi)皮細胞組和去全部細胞組間未見統(tǒng)計學(xué)差異(P0.05)。結(jié)論同種帶瓣主動脈經(jīng)去內(nèi)皮和傳統(tǒng)去細胞處理后,免疫原性均顯著下降,但去內(nèi)皮處理較傳統(tǒng)去細胞處理更好地保留了同種瓣的抗張性能。
[Abstract]:Objective to compare the biological characteristics and immunogenicity of endothelial homograft aortic homograft and traditional acellular aortic valve, and to explore the better preconditioning method of aortic homograft. Methods the rabbit aortic root tissue with valve was obtained and randomly divided into fresh group without special treatment. As a blank control group, the endothelial cells group (removal of endothelial cells by collagenase treatment) and the whole cell group (removal of all cell components in the tissue by traditional methods). The histomorphology of each group was observed by means of Masson's VG staining and scanning electron microscope. The biological characteristics of each group were evaluated by comparing the tissue thickness, tensile strength, tensile fracture rate and heat shrinkage temperature. The immunogenicity was evaluated by CD152 immunohistochemical staining and the determination of calcium content in grafts by atomic flame absorption spectrometry. Results the histomorphologic observation showed that both deendothelialization and acellular treatment had achieved the ideal acellular purpose. There was no significant difference in the thickness of the three tissues and the heat shrinkage temperature (P0.05). Compared with the fresh group, the tension resistance of the endothelial cells group did not change significantly (P0.05), the tensile performance of all the cells group decreased significantly (P0.01). The results of HE staining showed that the inflammatory reaction in the fresh group was the most intense, and a large number of inflammatory cells infiltrated after the embedding, and the inflammatory response in the endothelial cell removed group was significantly decreased compared with the fresh group. The number of inflammatory cells decreased significantly, and the inflammatory response was the weakest in all the cells group, with only a small number of inflammatory cells infiltrating at the edge of the tissue. Immunohistochemical observation showed that the expression of CD152 was significantly higher in the endothelium-free group and the total cell group than that in the control group (P0.05), but there was no significant difference between the endothelial cell removed group and the total cell removed group (P0.05). The results of calcium content analysis showed that the increase of calcium salt was significantly lower in the endothelium-free group and the total cell group than in the fresh group (P0.05), but there was no statistical difference between the endothelium-free group and the total cell group (P0.05). Conclusion the immunogenicity of aortic homograft was significantly decreased after both the removal of endothelium and the traditional decellularization of aorta, but the anti-tensile properties of the homograft were better preserved after the removal of endothelium than that of the conventional acellular treatment.
