【摘要】：目的評估介入治療急性肝動脈血栓形成的近期與遠(yuǎn)期療效。方法對復(fù)旦大學(xué)附屬中山醫(yī)院2003年3月至2015年10月出現(xiàn)的34例急性肝動脈血栓形成患者的介入處理方式和長期隨訪資料進(jìn)行回顧性分析。結(jié)果 34例急性肝動脈血栓形成患者均行經(jīng)肝動脈置管尿激酶溶栓治療。其中21例溶栓過程中聯(lián)合支架植入,3例合并脾動脈盜血行脾動脈栓塞。技術(shù)和臨床成功率均為91%(31/34)。2例出現(xiàn)與介入操作相關(guān)的并發(fā)癥。肝動脈1年、2年、3年及5年通暢率分別為82%、73%、57%和57%�；颊叩臒o阻塞中位生存期達(dá)94個月。結(jié)論介入治療急性肝動脈血栓形成獲得了良好的近期與遠(yuǎn)期療效,可作為肝移植術(shù)后急性肝動脈血栓形成的首選治療方式。
[Abstract]:Objective to evaluate the short-term and long-term outcomes of interventional therapy for acute hepatic artery thrombosis. Methods the interventional management and long-term follow-up data of 34 patients with acute hepatic artery thrombosis from March 2003 to October 2015 in Zhongshan Hospital affiliated to Fudan University were retrospectively analyzed. Results 34 patients with acute hepatic artery thrombosis were treated with urokinase via hepatic artery. During thrombolytic therapy, 3 cases of splenic artery embolism were performed with combined stent implantation and splenic artery steal. The technical and clinical success rates were 91.1 / 34.2 cases with complications related to interventional procedures. The patency rate of hepatic artery for 1 year, 2 years, 3 years and 5 years was 82% and 57%, respectively. The median non-obstructive survival was 94 months. Conclusion Interventional therapy for acute hepatic artery thrombosis has good short-term and long-term curative effect and can be used as the first choice of treatment for acute hepatic artery thrombosis after liver transplantation.
