【摘要】：目的評價(jià)股骨近端鎖定加壓鋼板(PFLCP)內(nèi)固定治療老年外側(cè)壁危險(xiǎn)型股骨粗隆間骨折的臨床療效。方法回顧性分析自2009-06—2013-10采用PFLCP內(nèi)固定治療72例老年外側(cè)壁危險(xiǎn)型股骨粗隆間骨折,術(shù)后依據(jù)Harris評分評價(jià)患者髖關(guān)節(jié)功能恢復(fù)情況。結(jié)果所有患者均獲得隨訪12~22個(gè)月,平均14.3個(gè)月。骨折均愈合,愈合時(shí)間13~21周,平均17.1周。術(shù)后1年療效依據(jù)Harris評分評定:優(yōu)54例,良10例,可3例,差5例,優(yōu)良率88.9%。出現(xiàn)髖內(nèi)翻畸形2例,髖外展無力1例,大粗隆頂端骨塊上移1例。無股骨頭切割、內(nèi)固定斷裂及骨折不愈合等并發(fā)癥發(fā)生。結(jié)論P(yáng)FLCP治療老年外側(cè)壁危險(xiǎn)型股骨粗隆間骨折符合生物力學(xué)特性,骨折愈合率高,功能恢復(fù)佳,并發(fā)癥少,是治療此類骨折安全有效的方法。
[Abstract]:Objective to evaluate the clinical effect of PFLCP-based internal fixation of proximal femoral locking compression plate in the treatment of elderly patients with perilous intertrochanteric fracture of the lateral wall of femur. Methods A retrospective analysis was made on 72 elderly patients with perilous intertrochanteric fracture of the lateral wall treated with PFLCP internal fixation since 2009-06-2013-10. The recovery of hip joint function was evaluated according to the Harris score. Results all the patients were followed up for 12- 22 months with an average of 14.3 months. The healing time was 1321 weeks (mean 17.1 weeks). According to Harris score, 54 cases were excellent, 10 cases were good, 3 cases were fair, 5 cases were poor, and the excellent and good rate was 88.9%. There were 2 cases of coxa varus deformity, 1 case of hip abduction weakness and 1 case of upward movement of bone mass at the top of great trochanter. There were no complications such as femoral head cutting, internal fixation fracture and fracture nonunion. Conclusion PFLCP is a safe and effective method for the treatment of peril femoral intertrochanteric fractures of the lateral wall of the elderly. The fracture healing rate is high, the functional recovery is good, and the complications are less.
