【摘要】：目的探討延長腹腔鏡手術時間對人體腹膜間皮細胞及纖維蛋白溶解活性的影響。方法對16例擬行腹腔鏡手術(腹腔鏡組)和21例開腹手術(開腹組)的高中位直腸癌患者進行前瞻性研究。在手術開始及隨后45、90、120min采集壁腹膜標本,采用ELISA法測定腹膜組織中組織型纖溶酶原激活物(tPA)和纖溶酶原激活物抑制劑(PAI-1)水平;培養(yǎng)大網(wǎng)膜間皮細胞,用乳酸脫氫酶法測定間皮細胞損傷情況,MTT法檢測細胞增殖活性。結果腹腔鏡組腹膜組織中PAI-1水平于手術45、90、120min時顯著低于開腹組,開腹組tPA水平在手術45min顯著降低,但在腹腔鏡組未有明顯改變;延長手術時間可引起腹腔鏡組腹膜間皮細胞乳酸脫氫酶活性增加、增殖活性降低。結論腹腔鏡手術通過抑制PAI-1增量調(diào)節(jié),延長腹膜纖溶作用,減少術后并發(fā)腹膜粘連形成。腹腔鏡延長手術時間可增加間皮細胞損傷、降低增殖活性。
[Abstract]:Objective to investigate the effect of prolonged laparoscopic surgery on peritoneal mesothelial cells and fibrinolytic activity. Methods A prospective study was carried out in 16 patients with middle and high rectal cancer undergoing laparoscopic surgery (laparoscopic group) and 21 patients undergoing open surgery (open group). The parietal peritoneal specimens were collected at the beginning of operation and 45 ~ 90min after operation. The levels of tissue plasminogen activator (TPA) and plasminogen activator inhibitor (PAI-1) in peritoneal tissue were measured by ELISA method, and the mesothelial cells of greater omentum were cultured. The damage of mesothelial cells was measured by lactate dehydrogenase assay. Results the level of PAI-1 in peritoneal tissue of laparoscopy group was significantly lower than that of open group at 45 ~ 90120min. The tPA level in laparotomy group was significantly lower than that in open group, but there was no significant change in laparoscopy group. Prolonging the operation time could increase the activity of lactate dehydrogenase and decrease the proliferative activity of peritoneal mesothelial cells in laparoscopic group. Conclusion Laparoscopic surgery can inhibit PAI-1 increment, prolong peritoneal fibrinolysis and reduce postoperative peritoneal adhesion. Prolonging the operation time by laparoscopy can increase the injury of mesothelial cells and decrease the proliferative activity.
