【摘要】：目的:探討膝關(guān)節(jié)骨性關(guān)節(jié)炎患者全膝關(guān)節(jié)置換術(shù)后膝關(guān)節(jié)局部皮溫及術(shù)后康復(fù)效果相關(guān)指標(biāo)的變化規(guī)律,并對(duì)它們的相關(guān)性進(jìn)行統(tǒng)計(jì)學(xué)分析,從而探討全膝關(guān)節(jié)置換術(shù)后膝關(guān)節(jié)局部皮溫測定對(duì)術(shù)后康復(fù)的指導(dǎo)意義,并進(jìn)一步探究全膝關(guān)節(jié)置換術(shù)后膝關(guān)節(jié)局部皮溫測定這一檢查方式在臨床工作中是否有應(yīng)用及推廣的價(jià)值。方法:本研究選取2015年9月至2016年2月因膝關(guān)節(jié)骨性關(guān)節(jié)炎就診于青島大學(xué)附屬醫(yī)院關(guān)節(jié)外科并接受初次全膝關(guān)節(jié)置換術(shù)的94位更年期后女性患者,對(duì)她們術(shù)前和術(shù)后的膝關(guān)節(jié)局部皮溫以及與術(shù)后康復(fù)密切相關(guān)的理化指標(biāo)進(jìn)行監(jiān)測,研究它們的變化規(guī)律,并運(yùn)用統(tǒng)計(jì)學(xué)方法進(jìn)行相關(guān)性分析,根據(jù)結(jié)果討論全膝關(guān)節(jié)置換術(shù)后膝關(guān)節(jié)局部皮溫測定在臨床工作中的應(yīng)用價(jià)值。結(jié)果:根據(jù)各項(xiàng)研究指標(biāo)變化折線圖可知,全膝關(guān)節(jié)置換術(shù)后患者膝關(guān)節(jié)局部皮溫變化和其他觀察指標(biāo)的變化趨勢有內(nèi)在聯(lián)系。經(jīng)統(tǒng)計(jì)學(xué)分析,膝關(guān)節(jié)局部皮溫變化同患者外周血白細(xì)胞計(jì)數(shù)及術(shù)后患肢腫脹程度變化呈高度正相關(guān),同術(shù)后患肢肌力呈顯著負(fù)相關(guān)。結(jié)論:患者術(shù)后雙膝關(guān)節(jié)皮溫差的變化趨勢能夠提示患者術(shù)后炎性指標(biāo)、疼痛程度、患肢肌力、患肢膝關(guān)節(jié)活動(dòng)度以及患肢腫脹程度的變化趨勢。尤其在患者術(shù)后外周血白細(xì)胞計(jì)數(shù)、患肢肌力和患肢腫脹程度等方面具有較好的指導(dǎo)意義。因此,全膝關(guān)節(jié)置換術(shù)后膝關(guān)節(jié)局部皮溫的測定有很好的臨床應(yīng)用及推廣價(jià)值。
[Abstract]:Objective: to investigate the changes of local skin temperature of knee joint and rehabilitation effect after total knee arthroplasty in patients with osteoarthritis of the knee, and to analyze their correlation statistically. The purpose of this study was to explore the significance of local skin temperature measurement in postoperative rehabilitation after total knee arthroplasty. And further to explore whether the method of measuring the local skin temperature of knee joint after total knee arthroplasty has the value of application and popularization in clinical work. Methods: from September 2015 to February 2016, 94 postmenopausal female patients who were admitted to the Department of Arthroplasty in the affiliated Hospital of Qingdao University and received the first total knee arthroplasty were selected. The local skin temperature of the knee joint before and after operation and the physical and chemical indexes closely related to postoperative rehabilitation were monitored, their changes were studied, and the correlation analysis was carried out by statistical method. Results the clinical value of local skin temperature measurement after total knee arthroplasty was discussed. Results: according to the curve diagram of the changes of various indexes, the changes of local skin temperature of knee joint after total knee arthroplasty were closely related to the trend of other observed indexes. By statistical analysis, the change of local skin temperature of knee joint was highly positively correlated with the peripheral blood leukocyte count and the degree of swelling of the affected limb, and negatively correlated with the muscle strength of the affected limb after operation. Conclusion: the change trend of skin temperature difference between two knee joints after operation can indicate the change trend of inflammatory index, pain degree, muscle strength of affected limb, knee motion degree and swelling degree of affected limb. Especially in peripheral blood leukocyte count, muscle strength and swelling degree of affected limbs. Therefore, the measurement of local skin temperature after total knee arthroplasty has good clinical application and popularization value.
