【摘要】：目的觀察葡萄球菌感染的膝關(guān)節(jié)置換兔局部低強(qiáng)度超聲波刺激的效果。方法清潔級SD兔40只,采用膝關(guān)節(jié)試件(萬古霉素骨水泥制作)對兔膝關(guān)節(jié)行膝關(guān)節(jié)置換,術(shù)后隨機(jī)分為觀察組和對照組各20只。術(shù)后第3天取兩組兔于膝關(guān)節(jié)處緊貼膝關(guān)節(jié)后方穿刺注射金黃色葡萄球菌菌液0.1 m L1次后,觀察組予低強(qiáng)度超聲輻射,1次/d,連續(xù)5 d。對照組不做任何處理。術(shù)后第7天,采集兩組頸動脈血檢測兩組血清萬古霉素。術(shù)后第30天觀察兩組膝關(guān)節(jié)炎癥情況,術(shù)后第30天處死兔,觀察膝關(guān)節(jié)液金黃色葡萄球菌培養(yǎng)情況,觀察膝關(guān)節(jié)滑膜組織感染情況。結(jié)果術(shù)后第7天觀察組、對照組血清萬古霉素水平分別為(60.23±8.36)、(40.36±6.28)mmol/L,兩組比較,P0.05。術(shù)后第30天時觀察組、對照組炎癥評分分別為(2.13±0.08)、(3.12±1.02)分,兩組比較,P0.05。觀察組對照組膝關(guān)節(jié)附近、關(guān)節(jié)腔內(nèi)均可見炎癥,但觀察組炎癥范圍信號低,范圍小。觀察組金黃色葡萄球菌生長較少,甚至沒有菌落生長,對照組可見大量淡黃色的陽性葡萄球菌菌落。術(shù)后第30天時觀察組、對照組感染評分分別為(2.23±0.79)、(4.84±0.13)分,兩組比較,P0.05。結(jié)論局部低強(qiáng)度超聲波刺激可防治膝關(guān)節(jié)置換術(shù)后早期感染。
[Abstract]:Objective to observe the effect of local low-intensity ultrasonic stimulation in rabbits with staphylococcal infection. Methods knee joint replacement was performed in 40 SD rabbits with clean grade by using vancomycin bone cement. The rabbits were randomly divided into two groups: the observation group and the control group. On the third day after operation, the rabbits in both groups were injected with 0.1 m L1 staphylococcus aureus at the back of the knee joint. The observation group was treated with low intensity ultrasound irradiation for 5 days. No treatment was given in the control group. On the 7th day after operation, two groups of carotid blood samples were collected to detect vancomycin. The knee joint inflammation was observed on the 30th day after operation and the rabbits were killed on the 30th day after operation. The culture of staphylococcus aureus in the knee joint fluid was observed and the infection of synovial tissue of the knee joint was observed. Results on the 7th day after operation, the serum vancomycin levels in the observation group and the control group were 60.23 鹵8.36 鹵6.28 mmol / L, 40.36 鹵6.28 渭 mol / L, respectively, compared with the control group (P 0.05). On the 30th day after operation, the inflammatory scores of the control group were 2.13 鹵0.08 and 3.12 鹵1.02, respectively, compared with that of the control group (P 0.05). Inflammation was seen in the joint cavity near the knee joint in the observation group, but the range of inflammation was low and small in the observation group. The growth of Staphylococcus aureus in the observation group was less than that in the control group, and a large number of yellowish positive staphylococcus colonies were found in the control group. On the 30th day after operation, the infection score of the control group was 2.23 鹵0.79 and 4.84 鹵0.13, respectively, compared with that of the control group (P 0.05). Conclusion Local low-intensity ultrasound stimulation can prevent early infection after knee arthroplasty.
