【摘要】：目的:通過本研究揭示CTRP12在CAD中的變化及與七氟醚的關系篩查CTRP12作為預示冠心病臨床檢測指標的可能性,并明確CTRP12的心臟保護效應。方法:隨機選取健康體檢志愿者62例為對照組(N組),選取心內科經冠脈造影證實CAD患者67例為冠心病組(C組),選取心外科預備行非體外循環(huán)的冠脈旁路移植術(OPCABG)患者52例分為丙泊酚組(P組)和丙泊酚+七氟醚組(S組),所有受試者均需禁食8小時以上(手術組還需同時禁飲6至少小時)并于次日清晨采集靜脈血3~5ml,P組和S組須另外采集術后4h靜脈血各5ml,設定在1000轉/分的轉速離心15分鐘,用移液槍吸取上層血漿,分裝放入設定溫度在-70℃的冰箱凍存。記錄N組、C組的年齡、性別、吸煙率、飲酒率、糖尿病患病率、高血壓患病率、BMI、TC、TG、HDL-C、LDL-C、FBG以及CRP,并采用酶聯(lián)免疫吸附法檢測C組和N組的CTRP12、IL-1β;采用酶聯(lián)免疫吸附法檢測P組和S組術前P0、S0及術后P1、S1四個時間點的CTRP12濃度,并計算P0和P1,S0和S1的差值dP和dS。結果:所有數據用SPSS軟件分析。結果得出冠心病組CTRP12與降低的IL-1β、HDL-C和增高的BMI呈正相關。在N、C、P、S組受試對象中CTRP12血漿水平與高血壓患病率和BMI呈正相關,與HDL-C和IL-1β呈負相關。logistic回歸分析CTRP12是冠心病的獨立危險因素。與N組相比C組CTRP12濃度明顯升高,且兩者的差異有統(tǒng)計學意義。S組和P組術前CTRP12濃度無差異,P組CTRP12濃度在誘導前與術后4小時的差值dP低于S組誘導前與術后4小時的差值dS,且具有統(tǒng)計學差異。結論:目前的研究顯示,血清CTRP12濃度可以為預測冠心病患病率提供參考。七氟醚的心肌保護作用可能與CTRP12有關。
[Abstract]:Objective: to investigate the changes of CTRP12 in CAD and the relationship between CTRP12 and sevoflurane (sevoflurane) as a predictor of coronary heart disease (CHD), and to determine the cardioprotective effect of CTRP12. Methods: 62 healthy volunteers were randomly selected as control group, 67 patients with CAD confirmed by coronary angiography in cardiology were selected as group C, and cardiac surgery was performed with off-pump coronary artery bypass grafting (OPCABG). 52 patients were divided into propofol group (group P) and propofol sevoflurane group (group S). All subjects had to fast for more than 8 hours (the operation group had to stop drinking for at least 6 hours at the same time) and collected venous blood in the next morning. Group A should collect 5 ml venous blood 4 hours after operation and centrifuge for 15 minutes at a rotational speed of 1000 rpm / min.
