【摘要】：目的探討平臥位下改良前外側(cè)入路全髖關(guān)節(jié)置換術(shù)的技術(shù)要點以及療效。方法2013年8月-2016年8月采用平臥位下改良前外側(cè)入路全髖關(guān)節(jié)置換術(shù)321例,男170例,女151例,年齡在46~84歲,平均76歲,記錄切口長度、術(shù)中出血量、手術(shù)時間、術(shù)后下地時間、術(shù)后3月跛行率及Trendelenburg征陽性率、并發(fā)癥,隨訪髖關(guān)節(jié)Harris評分。結(jié)果切口長度平均為8.4cm(7.0~10.0)cm,術(shù)中出血量平均165ml(110~300)ml,手術(shù)時間平均40 min(30~50)min,術(shù)后平均下地行走時間為2.5d(2~5 d),術(shù)后3個月跛行率及Trendelenburg征陽性率3%,未出現(xiàn)脫位、假體位置不良情況,隨訪時間最短3月,最長達(dá)3年,平均Harris評分達(dá)94.5分。結(jié)論平臥位下改良前外側(cè)入路全髖關(guān)節(jié)置換術(shù)療效確切,創(chuàng)傷小、恢復(fù)快、值得臨床推廣。
[Abstract]:Objective to investigate the technique and effect of modified anterolateral approach for total hip arthroplasty under supine position.Methods from August 2013 to August 2016, 321 patients (170 males and 151 females) underwent total hip arthroplasty with modified anterolateral approach under supine position. The mean age was 76 years (46 ~ 84 years). The length of incision, the amount of intraoperative bleeding, the time of operation and the time of operation were recorded.The rate of claudication and the positive rate of Trendelenburg sign, complication, and Harris score of hip joint were followed up 3 months after operation.Results the average length of incision was 8.4 cm ~ 7.0 ~ 10.0 cm, the average volume of blood loss during operation was 165 ml / 11010 ~ (300) ml, the average operation time was 40 min ~ 30 ~ 50 min, the average walking time was 2.5d(2~5 d, the claudication rate and the positive rate of Trendelenburg sign were 3 ~ 3 months after operation, there was no dislocation, and the prosthesis position was poor.The minimum follow-up time was 3 months, the longest was 3 years, and the average Harris score was 94.5.Conclusion the modified anterolateral approach for total hip arthroplasty in supine position is effective, less traumatic and quick to recover. It is worth popularizing.
