【摘要】：【目的】Glidescope視頻喉鏡較直接喉鏡可以提供更好的暴露視野,但對于是否縮短插管時間、提高一次插管成功率、改善插管時血流動力學(xué)改變等方面仍有異議。本次研究目的在于比較評價GlideScope視頻喉鏡與普通Macintosh喉鏡在不同氣管插管情況中的效果�！痉椒ā客ㄟ^檢索Medline、Embase、Cochrane圖書館和一些參考書目,篩選符合納入標(biāo)準(zhǔn)的文獻(xiàn),按照隨機(jī)對照試驗(yàn)的質(zhì)量評價標(biāo)準(zhǔn)嚴(yán)格評價各個相關(guān)納入文獻(xiàn),對納入文獻(xiàn)的相關(guān)研究數(shù)據(jù)進(jìn)行提取:(1)插管時間和一次插管成功率(2)困難插管評分(IDS)、插管期間血流動力學(xué)改變和并發(fā)癥�！窘Y(jié)果】Glidescope喉鏡比Macintosh喉鏡成人經(jīng)鼻插管時間縮短[MD,-13.91s;95%CI(-23.60s,-4.22s)],但延長小兒插管時間[MD,7.64s,95%CI(2.78s,12.50s)]和成人經(jīng)口插管時間[MD,8.75s;95%CI(3.62s,13.89)],此外提高困難插管和初學(xué)者的一次插管成功率分別是[RR,3.70;95%CI(1.81,7.54)]和[RR 7.88,95%CI(2.95,21.07)]。Glidescope視頻喉鏡可以降低IDS,但對插管引起的并發(fā)癥和血流動力學(xué)無明顯改善�！窘Y(jié)論】GlideScope可視喉鏡與Macintosh直接喉鏡相比,能提高聲門暴露程度,降低困難插管評分,但不能縮短經(jīng)口和小兒氣管插管時間,對并發(fā)癥和血流動力學(xué)改變無明顯改善。對于初學(xué)者插管、困難氣道插管而言,Glidescope喉鏡一次插管成功率較高。此外,Glidescope喉鏡可以縮短成人經(jīng)鼻插管時間。
[Abstract]:Objective: Glidescope video laryngoscope can provide better exposed visual field than direct laryngoscope, but there are still some dissent on whether to shorten the intubation time, increase the success rate of one intubation, and improve the hemodynamic changes during intubation.The purpose of this study was to compare and evaluate the effects of GlideScope video laryngoscope and common Macintosh laryngoscope on different tracheal intubation.According to the quality evaluation criteria of randomized controlled trials, the relevant literature was strictly evaluated.The relevant data included in the literature were extracted from the data collected from the study. The time of intubation and the success rate of one intubation were extracted. (2) the score of difficult intubation, hemodynamic changes and complications during intubation. [results] Glidescope laryngoscope was better than Macintosh laryngoscope in adult nasal intubation.There was no significant improvement in hemodynamics and symptom. [conclusion] GlideScope visual laryngoscope is more effective than Macintosh direct laryngoscope.It can improve the glottic exposure and reduce the score of difficult intubation, but it can not shorten the time of tracheal intubation through mouth and children, and has no significant improvement on complications and hemodynamic changes.For beginner intubation, the success rate of Glidescope laryngoscope intubation is higher.In addition, Glidescope laryngoscope can shorten the time of nasal intubation in adults.
