【摘要】：目的探討近紅外光譜(NIRS)檢測對硬膜外及硬膜下血腫的臨床應(yīng)用價值。方法選取34例經(jīng)CT或MRI證實的硬膜外或硬膜下血腫患者(試驗組)及14名健康志愿者(對照組),采用中國科學(xué)院自動化研究所研發(fā)的NIRS設(shè)備進(jìn)行頭部檢測,評判是否存在顱內(nèi)出血。以CT或MRI診斷結(jié)果為"金標(biāo)準(zhǔn)",評價NIRS對硬膜外及硬膜下血腫的診斷效能。結(jié)果 NIRS診斷硬膜外及硬膜下血腫的敏感度、特異度、準(zhǔn)確率、陽性預(yù)測值及陰性預(yù)測值分別91.18%(31/34)、71.43%(10/14)、85.42%(41/48)、88.57%(31/35)及76.92%(10/13)。結(jié)論 NIRS在檢測硬膜外及硬膜下血腫方面具有較高的敏感度和陽性預(yù)測值,有助于臨床早期診斷和治療。
[Abstract]:Objective to evaluate the clinical value of NIR spectroscopy in the detection of epidural and subdural hematoma.Methods 34 patients with epidural or subdural hematoma confirmed by CT or MRI (experimental group) and 14 healthy volunteers (control group) were selected for head examination using NIRS equipment developed by Institute of Automation, Chinese Academy of Sciences.The presence of intracranial hemorrhage was assessed.The diagnostic efficacy of NIRS in the diagnosis of epidural hematoma and subdural hematoma was evaluated by CT or MRI.Results the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of NIRS in the diagnosis of epidural and subdural hematoma were 91.18 / 31 / 34 and 71.43 / 14 respectively.Conclusion NIRS has high sensitivity and positive predictive value in the detection of epidural hematoma and subdural hematoma, which is helpful for early clinical diagnosis and treatment.
