【摘要】：目的：本課題通過(guò)對(duì)比觀察,探討前路手術(shù)結(jié)合益氣活血通絡(luò)法治療頸椎病的臨床療效。方法：根據(jù)臨床癥狀和影像學(xué)表現(xiàn),將符合標(biāo)準(zhǔn)的62例頸椎病患者隨機(jī)分成試驗(yàn)組與對(duì)照組,每組各31例,均行頸椎前路椎體次全切除+椎間盤(pán)摘除+鈦網(wǎng)植骨融合鋼板內(nèi)固定術(shù)。術(shù)后對(duì)照組常規(guī)治療,試驗(yàn)組在常規(guī)治療基礎(chǔ)上,術(shù)后第3天予以服用益氣活血通絡(luò)法代表方加昧補(bǔ)陽(yáng)還五湯(免煎制劑,每天1劑,連服15天)。通過(guò)觀察兩組手術(shù)操作時(shí)間,術(shù)中出血量,術(shù)前及術(shù)后1、3個(gè)月神經(jīng)功能和軸性癥狀,術(shù)后3個(gè)月植骨融合等情況,應(yīng)用SPSS19.0統(tǒng)計(jì)學(xué)軟件對(duì)所得數(shù)據(jù)進(jìn)行分析,對(duì)比各指標(biāo)組內(nèi)和組間的差異,以P0.05表示差異有統(tǒng)計(jì)學(xué)意義。結(jié)果：術(shù)前神經(jīng)功能及軸性癥狀組間比較無(wú)明顯差異,術(shù)后1、3個(gè)月神經(jīng)功能組間比較,試驗(yàn)組優(yōu)于對(duì)照組；術(shù)后1個(gè)月軸性癥狀組間比較試驗(yàn)組優(yōu)于對(duì)照組,術(shù)后3個(gè)月軸性癥狀兩組之間差異無(wú)統(tǒng)計(jì)學(xué)意義。兩組病例在手術(shù)操作時(shí)間、術(shù)中出血量、術(shù)后3個(gè)月植骨融合情況等方面差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論：前路手術(shù)結(jié)合益氣活血通絡(luò)法治療頸椎病能夠促進(jìn)神經(jīng)功能恢復(fù),有效地改善術(shù)前軸性癥狀和降低術(shù)后軸性癥狀的發(fā)生率,提高手術(shù)效果,臨床優(yōu)良率達(dá)到100%,值得推廣。
[Abstract]:Objective: to explore the clinical effect of anterior approach combined with the method of supplementing qi and activating blood circulation and dredging collaterals in the treatment of cervical spondylosis by comparative observation. Methods: according to the clinical symptoms and imaging manifestations, 62 patients with cervical spondylosis were randomly divided into experimental group and control group with 31 cases in each group. On the basis of routine treatment, the experimental group was treated with Yiqi Huoxue Tongluo decoction on the third day after operation (no decoction, 1 dose per day, 15 days). By observing the operation time and the amount of blood loss during the operation, the experimental group was treated with Fubuyang Huanwu decoction. The neurological function and axonal symptoms before and 1, 3 months after operation, bone graft fusion at 3 months after operation and so on were analyzed by SPSS19.0 statistical software, and the differences among groups were compared. Results: there was no significant difference in preoperative nerve function and axonal symptoms between the two groups. After 1 month and 3 months after operation, the experimental group was better than the control group. One month after operation, there was no significant difference between the two groups in axial symptoms, but there was no significant difference between the two groups in three months after operation. The operation time and the amount of blood lost during the operation of the two groups were significantly higher than those in the control group, and there was no significant difference between the two groups. There was no significant difference in bone graft fusion in 3 months after operation (P 0.05). Conclusion: anterior approach combined with Yiqi Huoxue Tongluo method for cervical spondylosis can promote the recovery of nerve function. It can effectively improve the preoperative axial symptoms and reduce the incidence of postoperative axial symptoms, and improve the surgical effect. The clinical excellent and good rate is 100, which is worth popularizing.
