【摘要】：目的探討瑞芬太尼在中老年患者無(wú)痛支氣管鏡檢查中的作用。方法將120例行無(wú)痛支氣管鏡檢查的中老年患者隨機(jī)分為4組(n=30),A組用丙泊酚麻醉,B、C、D組用瑞芬太尼復(fù)合丙泊酚麻醉(瑞芬太尼輸注劑量:B組0.05μg·kg~(-1)·min~(-1),C組0.10μg·kg~(-1)·min~(-1),D組0.15μg·kg~(-1)·min~(-1))。觀察各組患者在檢查過(guò)程中的體動(dòng)反應(yīng)、血流動(dòng)力學(xué)變化、丙泊酚用量、檢查時(shí)間、蘇醒時(shí)間、離室時(shí)間及不良反應(yīng)。結(jié)果在檢查過(guò)程中,A組的術(shù)中體動(dòng)和嗆咳明顯高于其他3組(P0.05);在血流動(dòng)力學(xué)上,A組波動(dòng)較大,B、C、D組相對(duì)較平穩(wěn),但D組低血壓、心動(dòng)過(guò)緩、一過(guò)性呼吸抑制發(fā)生率均較其他3組高(P0.05);丙泊酚用量A組明顯高于其他3組(P0.05),B組高于C、D組(P0.05);在蘇醒時(shí)間和離室時(shí)間上B、C、D組明顯優(yōu)于A組(P0.05),C、D組短于B組(P0.05)。結(jié)論中老年患者在無(wú)痛支氣管鏡檢查中可使用瑞芬太尼,且以0.10μg·kg~(-1)·min~(-1)的瑞芬太尼復(fù)合異丙酚麻醉效果較好。
[Abstract]:Objective to investigate the role of remifentanil in painless bronchoscopy in middle-aged and elderly patients. Methods 120 middle-aged and elderly patients with painless bronchoscopy were randomly divided into 4 groups: group A was treated with propofol anesthesia and group D was treated with remifentanil combined with remifentanil. Propofol anesthesia (remifentanil 0.05 渭 g kg 路kg ~ (-1)) in group C (0.10 渭 g 路kg ~ (-1)) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1) 路min ~ (-1). Hemodynamic changes, propofol dosage, examination time, recovery time, Results the intraoperative body movement and choking cough in group A were significantly higher than those in other three groups (P 0.05), but the hemodynamics of group A was relatively stable, but that of group D was hypotension and bradycardia. The incidence of transient respiratory inhibition was significantly higher in group A than in other three groups, and the dosage of propofol in group A was significantly higher than that in group C (P 0.05), and in group B was higher than that in group C (P 0.05), and in the recovery time and time of absence from room, group B was significantly better than group A (P 0.05) than group B (P 0.05). Conclusion the dosage of propofol in group A is significantly higher than that in group B. Remifentanil may be used in painless bronchoscopy. The anesthetic effect of remifentanil combined with propofol was better than that of remifentanil (0.10 渭 g 路kg ~ (-1)) 路min ~ (-1) 路min ~ (-1).
