Meek皮膚移植與微粒皮膚移植修復大面積深度燒傷創(chuàng)面效果與臨床應用價值的Meta分析

【摘要】:背景:大面積深度燒傷病人大部分皮膚被破壞,燒傷創(chuàng)面暴露于外界,急需自體皮膚移植,但這類病人的自體移植皮源匱乏,Meek植皮與微粒皮植皮是救治這類燒傷病人的主要手段,近十年以來,關于Meek植皮術的臨床療效陸續(xù)被報道,但是與微粒皮植皮的臨床對照研究的優(yōu)劣勢存在爭議,這些研究大部分樣本量小,缺乏說服力。目的:Meek植皮術與微粒皮植皮術在修復大面積深度燒傷創(chuàng)面的臨床療效及應用價值進行系統(tǒng)性評價。方法:用計算機以“Meek micrografting technique”AND“microskin graft methods”等為檢索詞檢索PubMed(1990/2016-10)、Ovid(1966/2016-10)、Elsevier(1988/2016-10)、SPRINGER(1995/2016-10)、The Cochrane Library(1978/2016-10)數(shù)據庫,以“Meek植皮”和“微粒皮”及“燒傷/burn”為關鍵檢索詞檢索萬方數(shù)據庫(1982/2016)、維普中文科技期刊數(shù)據庫(1979/2016)和中國知網數(shù)據庫(CNKI,1979/2016),并挑選并收集與Meek植皮術與微粒皮植皮術相關治療大面積深度燒傷創(chuàng)面的臨床對照研究(CCT)。評價納入研究的文獻質量,關鍵資料收集后,使用RevMan 5.1軟件行Meta分析。結果與結論:共納入8個臨床對照研究,包含302例大面積燒傷患者。相比于微粒皮植皮組,Meek植皮組在皮片存活率顯著增加,在創(chuàng)面愈合時間、皮片融合時間、手術操作時間及1%燒傷面積治療費用均明顯降低,表明Meek植皮術比微粒皮植皮術更能明顯增加皮片存活率并促進創(chuàng)面愈合,縮短手術操作時間,并且減少治療費用,在臨床應用價值更加有優(yōu)勢。
[Abstract]:Background: large area deep burn patients are mostly damaged, the burn wounds are exposed to the outside world, and autologous skin transplantation is urgently needed. However, Meek skin grafts and microskin grafts are the main methods to treat these burn patients. In the past ten years, the clinical effects of Meek skin grafting have been reported one after another. But there is controversy about the merits and demerits of clinical comparative studies with microskin grafts, most of which are small in size. Objective: to evaluate systematically the clinical efficacy and application value of Meek Meek skin grafting and microskin skin grafting in the repair of large area deep burn wounds. Methods: the computer was used to search the database of the Cochrane Library 1978-10 using "Meek micrografting technique" AND "microskin graft methods". Using "Meek skin grafting" and "Particle skin" and "Burn / Burn" as the key keywords to search Wanfang Database / 1982 / 2016, Weipu Chinese Science and Technology Journal Database / 1979 / 2016) and China knowledge Network Database to select and collect the images of Meek skin grafting and Particle skin grafting. A clinical controlled study on the treatment of large area deep burn wounds by Guan. Evaluation of the quality of literature included in the study. After the key data were collected, Meta analysis was performed with RevMan 5.1 software. Results and conclusion: eight clinical controlled studies were conducted, including 302 patients with large area burn. The survival rate of skin grafts in the Meek skin graft group was significantly higher than that in the microskin graft group. The time of wound healing, the time of skin graft fusion, the time of operation and the cost of treatment of 1% burn area were significantly decreased. The results showed that Meek skin grafting could significantly increase the survival rate of skin grafts and promote wound healing. Shortening the time of operation and reducing the cost of treatment is more valuable in clinical application.
