【摘要】：目的 探討腹腔鏡袖狀胃切除術(shù)(laparoscopic sleeve gastrectomy,LSG)對肥胖及肥胖合并疾病的療效及安全性。方法回顧性分析首都醫(yī)科大學(xué)附屬北京天壇醫(yī)院于2012年7月至2015年9月接受腹腔鏡胃袖狀切除手術(shù)的26例單純性肥胖病人臨床資料。記錄術(shù)后3、6、12和18個月時體質(zhì)量指數(shù)(body mass index,BMI)及多項生物化學(xué)指標等。結(jié)果 所有病人均順利完成LSG術(shù),無中轉(zhuǎn)開腹。手術(shù)時間50~170 min,平均(79.7±4.7)min;術(shù)中出血10~50m L,平均(24.6±1.8)m L;術(shù)后住院日為5~13 d,平均(5.9±0.3)d;術(shù)后腹腔積液、腹腔出血各1例,無二次手術(shù)病例。術(shù)后3、6、12、18個月時,多余體質(zhì)量減少分別為54.9%、62.7%、80.1%和68.4%,隨訪3~36個月未出現(xiàn)不可控制的體質(zhì)量下降等惡性營養(yǎng)不良。隨訪期間出現(xiàn)1例(3.8%)膽囊結(jié)石、2例(7.6%)缺鐵性貧血、2例(7.6%)胃食管反流癥狀。術(shù)后肥胖相關(guān)疾病如糖尿病、高血壓、脂肪肝、高脂血癥等均有明顯改善。結(jié)論 腹腔鏡袖狀胃切除術(shù)治療單純性肥胖癥減重效果確切、安全,但其與胃旁路手術(shù)等其他減重手術(shù)的效果對比尚需進一步研究。
[Abstract]:Objective to investigate the efficacy and safety of laparoscopic sleeve gastrectomy (LSGs) in the treatment of obesity and obesity complicated with diseases. Methods from July 2012 to September 2015, Beijing Temple of Heaven Hospital affiliated to Capital Medical University was treated with abdominal cavity. The clinical data of 26 patients with simple obesity undergoing endoscopic gastrectomy were recorded. Body mass index (BMI), body mass index (BMI) and biochemical indexes were recorded at 12 and 18 months after operation. Results LSG was successfully performed in all patients. There was no conversion to laparotomy. The operative time was 50 ~ 170 min (mean 79.7 鹵4.7 min), the intraoperative bleeding was 100.50ml (24.6 鹵1.8mL), the postoperative hospitalization time was 50.13 days (average 5.9 鹵0.3d), the postoperative peritoneal effusion was 1 case, and there was no secondary operation. The reduction of excess body mass was 62.70.1% and 68.4%, respectively. During the follow-up period of 3 ~ 36 months, there was no uncontrollable body mass loss and other malignant malnutrition. During the follow-up period, 1 case had 3. 8) gallstone and 2 cases had iron deficiency anemia (2 cases) with stomach and esophagus reflux symptoms. Postoperative obesity-related diseases such as diabetes, Conclusion Laparoscopic sleeve gastrectomy is effective and safe in the treatment of simple obesity. But the comparison of its effect with other weight-loss surgery, such as gastric bypass surgery, needs further study.
