【摘要】：目的探討右美托咪定復(fù)合瑞芬太尼靜脈恒速輸注用于心房顫動(房顫)導(dǎo)管射頻消融術(shù)中鎮(zhèn)靜和鎮(zhèn)痛的效果。方法選擇行房顫導(dǎo)管射頻消融術(shù)患者80例,用隨機(jī)數(shù)字表法分為右美托咪定復(fù)合瑞芬太尼組(DR組)和瑞芬太尼組(R組)。DR組在手術(shù)開始前10 min輸注右美托咪定1μg/kg,10 min泵完,隨后以0.3μg/(kg·h)持續(xù)泵注至手術(shù)結(jié)束。R組輸注等量生理鹽水。手術(shù)前兩組患者均靜脈持續(xù)泵注瑞芬太尼0.05μg/(kg·min)。記錄入手術(shù)室時(T0)、手術(shù)開始后10 min(T1)、30 min(T2)、60 min(T3)、120 min(T4)及術(shù)畢(T5)時的平均動脈壓、心率、呼吸、血氧飽和度和Ramsay鎮(zhèn)靜評分。同時記錄兩組患者術(shù)中追加瑞芬太尼的劑量、停藥后清醒時間、患者和手術(shù)醫(yī)師的滿意度以及術(shù)中不良事件的發(fā)生率。結(jié)果 DR組的Ramsay鎮(zhèn)靜評分在手術(shù)后各時間點(diǎn)明顯高于R組(P0.05);術(shù)中追加的瑞芬太尼的劑量明顯低于R組(P0.05);患者的滿意度評分和手術(shù)醫(yī)師的滿意度評分明顯高于R組(P0.05)。DR組心率減慢的發(fā)生率為12.5%,R組未觀察到心率減慢的發(fā)生。結(jié)論右美托咪定復(fù)合瑞芬太尼靜脈恒速輸注用于房顫導(dǎo)管射頻消融術(shù)具有較好的鎮(zhèn)靜和鎮(zhèn)痛效果、更好的患者和手術(shù)醫(yī)師滿意度。
[Abstract]:Objective to investigate the effect of intravenous intravenous infusion of dexmetomidine and remifentanil on sedation and analgesia in atrial fibrillation (AF) catheter radiofrequency ablation. Methods 80 patients with atrial fibrillation catheter radiofrequency ablation were selected. The patients were randomly divided into two groups: dexmetomidine combined with remifentanil group (Dr group) and remifentanil group (group R). Dexmetomidine 1 渭 g / kg was infused 10 min before operation. 10 min pump finished. After that, 0.3 渭 g / kg 路h of remifentanil was continuously pumped to the end of operation. Group R was infused with the same amount of normal saline. Before the operation, remifentanil 0.05 渭 g / kg 路min was continuously injected intravenously in both groups. ... recorded in the operating room (T0). The mean arterial pressure, heart rate and respiration were measured 10 min after operation and 30 min after operation. Blood oxygen saturation and Ramsay sedation score. The dosage of remifentanil was recorded in both groups, and the waking time after withdrawal was recorded. Results the Ramsay sedation score of Dr group was significantly higher than that of R group at each time point (P 0.05). The dose of remifentanil added during operation was significantly lower than that in group R (P 0.05). The patients' satisfaction score and the surgeon's satisfaction score were significantly higher than that of R group (P 0.05). Dr group had a lower rate of heart rate (12.5%). Conclusion right metoimidine combined with remifentanil has a good sedative and analgesic effect in atrial fibrillation catheter radiofrequency ablation. Better patient and surgeon satisfaction.
