【摘要】：[目的]探討有限內(nèi)固定聯(lián)合鉸鏈?zhǔn)酵夤潭ㄖ委熤怅P(guān)節(jié)"三聯(lián)征"的早期臨床療效。[方法]2012年8月~2015年8月,采用肘關(guān)節(jié)內(nèi)、外側(cè)聯(lián)合入路切開復(fù)位內(nèi)固定輔助鉸鏈?zhǔn)酵夤潭苤委?2例肘部損傷"三聯(lián)征"患者,男10例,女2例;年齡19~47歲,平均30.40歲;左側(cè)3例,右側(cè)9例。橈骨頭骨折Mason分類:Ⅱ型7例,Ⅲ型5例。尺骨冠狀突骨折按Regan—Morrey分類:I型2例,Ⅱ型6例,Ⅲ型4例。根據(jù)Mayo肘關(guān)節(jié)功能評(píng)分(MEPS)標(biāo)準(zhǔn)評(píng)定療效。[結(jié)果]12例患者術(shù)后獲6~13個(gè)月(平均9.50個(gè)月)隨訪。末次隨訪時(shí),肘關(guān)節(jié)屈伸及前臂旋轉(zhuǎn)活動(dòng)度分別平均為128°(119°~142°)和138°(127°~148°);MEPS評(píng)分為87~100分,平均96分,其中11例評(píng)定為優(yōu),1例為良,優(yōu)良率100%,無一例發(fā)生肘關(guān)節(jié)不穩(wěn)定,均未出現(xiàn)異位骨化及關(guān)節(jié)僵硬。[結(jié)論]有限內(nèi)固定聯(lián)合鉸鏈?zhǔn)酵夤潭ㄖ委熤怅P(guān)節(jié)"三聯(lián)征"可重建肘關(guān)節(jié)骨性結(jié)構(gòu),恢復(fù)其穩(wěn)定性,早期進(jìn)行功能鍛煉,減少肘關(guān)節(jié)功能丟失。
