【摘要】：目的觀察扁平形硬膜外脊膜瘤的影像學(xué)表現(xiàn)。方法回顧性分析8例經(jīng)手術(shù)病理證實(shí)的扁平形硬膜外脊膜瘤的影像學(xué)表現(xiàn)表現(xiàn),并進(jìn)行文獻(xiàn)復(fù)習(xí)。結(jié)果 7例硬膜外脊膜瘤位于頸椎管,1例位于胸椎管,累及2~4個(gè)椎體節(jié)段;8例病灶均呈扁平形、以增厚的硬膜為基底,伴硬膜外腫塊形成;5例腫瘤經(jīng)椎間孔向硬膜外延伸、相應(yīng)水平椎間孔擴(kuò)大;椎管均均勻變窄,1例伴脊髓水腫,1例伴脊髓變性。CT顯示病灶呈等密度,6例硬膜可見(jiàn)條片狀鈣化。MRI顯示病灶于軸位圖像上均呈半環(huán)形,包繞脊髓,T1WI上呈等(n=5)或低(n=3)信號(hào),T2WI上呈等(n=3)或低信號(hào)(n=5);增強(qiáng)后呈中等(n=2)或明顯(n=6)強(qiáng)化,其中7例強(qiáng)化均勻,1例不均勻。結(jié)論扁平形硬膜外脊膜瘤好發(fā)于頸椎管,可累及多個(gè)椎體節(jié)段,以硬膜增厚、鈣化并硬膜外腫塊形成為其特征。
[Abstract]:Objective to observe the imaging features of flat epidural meningioma.Methods the imaging findings of 8 cases of flat epidural meningioma confirmed by surgery and pathology were analyzed retrospectively and the literatures were reviewed.Results 7 cases of epidural meningioma were located in the cervical spinal canal and 1 case was located in the thoracic spinal canal. The lesions were flat in 8 cases involving 2 ~ 4 segments of vertebral body. The thickened dura mater was taken as the base, and 5 cases of tumors extended through the intervertebral foramen to the epidural canal with the formation of epidural mass.The corresponding horizontal intervertebral foramina was enlarged and the spinal canal narrowed evenly in 1 case with spinal cord edema and 1 case with spinal cord degeneration. Ct showed that the lesion presented isodensity and 6 cases showed strip calcification in dural. MRI showed that the lesion was semi-annular on axial image.The signal intensity on T _ 2WI was isochromatic or hypointense on T _ 1WI), or on T _ 2WI, the signal was isomorphous 3) or hypointense on T _ 2WI. After T _ 2WI, the signal intensity was moderate (n = 2) or obvious (n = 6)) enhancement, in which 7 cases showed homogeneous enhancement and 1 case was uneven.Conclusion the flattened epidural meningioma is prone to occur in the cervical spinal canal and may involve multiple segments of the vertebral body, characterized by thickening of the dura dural, calcification and formation of the epidural mass.
