新西蘭大白兔髓核組織在自身椎管內(nèi)組織學(xué)和免疫學(xué)改變的研究

【摘要】: 目的:制作游離型腰椎間盤(pán)突出動(dòng)物模型,觀察并探討游離髓核組織重吸收的可能機(jī)制,為臨床保守治療某些類(lèi)型腰椎間盤(pán)突出癥提供新的思路和依據(jù)。 設(shè)計(jì)、時(shí)間、地點(diǎn):隨機(jī)對(duì)照動(dòng)物實(shí)驗(yàn),2009年10月-2010年3月在延邊大學(xué)解剖學(xué)教研部動(dòng)物實(shí)驗(yàn)室及延邊大學(xué)醫(yī)院臨床病理科完成。 材料:健康雄性新西蘭大白兔90只,隨機(jī)分為三組:模型組60只,假模型組24只,對(duì)照組6只。 方法:模型組前方入路制作游離型腰椎間盤(pán)突出動(dòng)物模型,假模型組取相同手術(shù)入路,但僅暴露至纖維環(huán)而不做后續(xù)處理,對(duì)照組不做任何處理。模型組及假模型組分別于術(shù)后2、4、6、8、10、12周分批處死動(dòng)物,取手術(shù)區(qū)間椎間盤(pán)標(biāo)本。用免疫組化方法觀察髓核組織中CD68+巨噬細(xì)胞浸潤(rùn)情況。 結(jié)果:模型組移植的髓核組織不同程度縮小甚至消失,所有移植標(biāo)本內(nèi)均見(jiàn)CD68+巨噬細(xì)胞浸潤(rùn)隨時(shí)間延長(zhǎng)而逐漸減少,假模型組僅纖維環(huán)外緣見(jiàn)少量CD68+巨噬細(xì)胞浸潤(rùn),其髓核及對(duì)照組髓核均未見(jiàn)CD68+巨噬細(xì)胞浸潤(rùn)。 結(jié)論:(1)運(yùn)用前方手術(shù)入路可建立游離型腰椎間盤(pán)突出動(dòng)物模型。(2)游離髓核組織在硬膜外腔可逐漸被機(jī)體重吸收。(3)CD68+巨噬細(xì)胞浸潤(rùn)在游離髓核組織重吸收過(guò)程中起到重要作用。
[Abstract]:Objective: to make an animal model of free lumbar intervertebral disc herniation, observe and explore the possible mechanism of reabsorption of free nucleus pulposus tissue, so as to provide new ideas and basis for the conservative treatment of some types of lumbar intervertebral disc herniation.
Design, time, place: randomized controlled animal experiments, completed in March -2010 October 2009 in the animal laboratory of the Department of anatomy, Yanbian University and the clinicopathological Department of the Yanbian University hospital.
Materials: 90 healthy male New Zealand white rabbits were randomly divided into three groups: model group 60, false model group 24 and control group 6.
Methods: the animal model of the free lumbar intervertebral disc herniation was made in front of the model group. The false model group was taken the same surgical approach, but only the fibrous ring was exposed without subsequent treatment. The control group did not do any treatment. The model group and the fake model group died at the 2,4,6,8,10,12 weeks after the operation, and the intervertebral disc specimens were taken from the operation interval. Histochemical method was used to observe the infiltration of CD68+ macrophages in nucleus pulposus.
Results: the nucleus pulposus tissue of the model group reduced and even disappeared in different degrees. The infiltration of CD68+ macrophage in all the transplanted specimens gradually decreased with time. Only a small amount of CD68+ macrophage infiltration in the false model group was seen in the outer margin of the false model group, and the nucleus pulposus and the nucleus pulposus of the control group had no CD68+ macrophage infiltration.
Conclusions: (1) an animal model of free lumbar intervertebral disc herniation can be established by using the anterior approach. (2) the free nucleus pulposus can be gradually absorbed by the body in the epidural cavity. (3) the infiltration of CD68+ macrophages plays an important role in the reabsorption of free nucleus pulposus tissue.
