【摘要】： 目的： 建立2μm激光離體動(dòng)物組織操作培訓(xùn)模型,并對(duì)應(yīng)用該模型進(jìn)行操作技能培訓(xùn)進(jìn)行評(píng)價(jià)。 對(duì)象與方法： 解凍的冰凍離體豬心臟被固定在離體動(dòng)物組織培訓(xùn)模型培訓(xùn)箱內(nèi)。無(wú)2μm激光操作經(jīng)驗(yàn)的低年資外科醫(yī)師10名,分2階段接受使用2μm激光前列腺切割汽化操作鏡,在離體動(dòng)物組織模型中進(jìn)行對(duì)豬心肌腱索切割和汽化操作技術(shù)培訓(xùn)并分別記錄操作時(shí)間。期間應(yīng)用生理鹽水沖洗以保持術(shù)野清晰。第1階段前對(duì)受訓(xùn)者進(jìn)行豬心臟解剖、離體動(dòng)物組織模型中進(jìn)行切割和汽化操作要求的講解,第2階段前對(duì)受訓(xùn)者進(jìn)行2μm激光相關(guān)理論和操作培訓(xùn)。分析比較2階段基本技術(shù)操作時(shí)間變化。 結(jié)果： 應(yīng)用冰凍離體豬心臟建立2μm激光離體動(dòng)物組織培訓(xùn)模型經(jīng)濟(jì)、可行。受訓(xùn)者對(duì)離體動(dòng)物組織培訓(xùn)模型接受良好。操作空間大小類似應(yīng)用激光治療BPH時(shí)前列腺尿道部的空間,足夠進(jìn)行激光操作培訓(xùn)。2階段培訓(xùn)順利進(jìn)行,無(wú)危險(xiǎn)性意外事件發(fā)生。10名受訓(xùn)者中2名缺席第2輪操作,共收集到8組數(shù)據(jù)。8名受訓(xùn)者第1階段切割時(shí)間、汽化時(shí)間以及總操作時(shí)間分別為166±94s,224±81s,390±145s；接受培訓(xùn)后的第2階段切割時(shí)間、汽化時(shí)間以及總操作時(shí)間分別為75±28s,105±28s,180±46s。2階段比較差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。 結(jié)論： 建立2μm激光離體動(dòng)物組織操作培訓(xùn)模型進(jìn)行相關(guān)技術(shù)培訓(xùn)可行。接受理論與操作指導(dǎo)并應(yīng)用本模型培訓(xùn)可以顯著提高受訓(xùn)者基本操作技術(shù)如切割和汽化。
[Abstract]:Objective: A 2 渭 m laser in vitro animal tissue training model was established, and the application of the model to the training of operation skills was evaluated. Objects and methods: Thawed frozen isolated pig hearts were fixed in an in vitro animal tissue training model training kit. Ten young surgeons without 2 渭 m laser operating experience were treated with 2 渭 m laser vaporization operation mirror in 2 stages. In vitro animal tissue model, the pig cardiac tendon cord cutting and vaporization techniques were trained and the operation time was recorded. Use saline to keep the surgical field clear. In the first stage, the pig heart was dissected, and the requirements of cutting and vaporizing were explained in the animal tissue model in vitro. In the second stage, the trainees were trained in the related theory and operation of 2 渭 m laser. Analyze and compare the change of operation time of basic technology in two stages. Results: Using frozen isolated pig heart to establish 2 渭 m laser in vitro animal tissue training model is economical and feasible. The trainees received good training models of in vitro animal tissues. The size of the operation space was similar to that of the urethral part of the prostate treated with laser, which was sufficient for the training of laser operation. 2 out of 10 trainees were absent from the second round operation. A total of 8 groups of data were collected. The cutting time, vaporization time and total operating time of the first stage of 8 groups of trainees were 166 鹵94s, 224 鹵81s, 390 鹵145s, respectively. The vaporization time and the total operating time were 75 鹵28sL, 105 鹵28sL and 180 鹵46s.2, respectively. The difference was statistically significant (P 0.01). Conclusion: The establishment of 2 渭 m laser in vitro animal tissue training model is feasible. Receiving theoretical and operational guidance and applying this model can significantly improve the basic operating techniques such as cutting and vaporization.
