【摘要】： 目的為進行移植免疫研究的需要,建立一種穩(wěn)定可靠的大鼠腎移植動物模型。并探討兩種術式的優(yōu)缺點。方法80只SD大鼠分兩組進行,A組(供受體各20只)采用大鼠腎動靜脈的端端吻合,輸尿管帶部分膀胱壁與受體膀胱吻合法,即原位腎移植組;B組(供受體各20只)是將供體腎動脈相連的腹主動脈及腎靜脈以端側吻合的方式移植到受體腎動脈稍下方的腹主動脈和下腔靜脈。通過分析兩組移植后大鼠死亡率、手術時間、及并發(fā)癥發(fā)生情況以得出穩(wěn)定可靠的動物模型建立方法。并了解腎動靜脈吻合方式對移植大鼠的影響。結果A組20只移植大鼠平均存活21.4d,平均手術時間121.05±15.02min,術后常見的并發(fā)癥主要有腎血管的狹窄和血栓、吻合口出血、輸尿管壞死等。B組20大鼠平均存活24.4d,平均手術時間127.7±11.98min,術后最常見并發(fā)癥為下身癱瘓、腎血管血栓、吻合口出血、輸尿管狹窄或尿瘺、壞死等。A組與B組在術后各時間段的死亡率以及平均手術時間無明顯差別(P0.05),A組的動脈吻合時間明顯長于B組(P0.05),而靜脈吻合時間小于B組(P0.05),均具有統計學意義;輸尿管帶部分膀胱瓣的吻合時間無顯著性差異(P0.05)。結論大鼠腎動靜脈端端吻合法的原位移植術能建立一種穩(wěn)定可靠的動物模型,其具有并發(fā)癥少,愈合快,而成功率基本相當的優(yōu)點,并且供腎切取時腎靜脈預留結扎線的方法使腎靜脈吻合時間明顯縮短。
[Abstract]:Objective in order to carry out the research of transplantation immunity, A stable and reliable animal model of renal transplantation in rats was established and the advantages and disadvantages of the two methods were discussed. Methods 80 SD rats were divided into two groups and received end-to-end anastomosis of renal arteriovenous vessels in group A (20 recipients each). Partial bladder wall of ureteral band and recipient bladder anastomosis, In orthotopic renal transplantation group B (20 recipients each), the abdominal aorta and the renal vein connected with the donor renal artery were transplanted to the abdominal aorta and the inferior vena cava slightly below the recipient renal artery by end-to-side anastomosis. Group A, the mortality rate of rats after transplantation, Operation time, In order to establish a stable and reliable animal model, and to understand the effect of renal arteriovenous anastomosis on the transplanted rats, the average survival time of 20 rats in group A was 21.4 days and the average operative time was 121.05 鹵15.02 min. The main complications were renal vascular stenosis and thrombosis. The average survival time of 20 rats in group B was 24.4 days, and the average operative time was 127.7 鹵11.98 min. The most common postoperative complications were lower body paralysis, renal vascular thrombosis, anastomotic bleeding, ureteral stenosis or urinary fistula. There was no significant difference in mortality and mean operative time between group A and group B after operation. The time of arterial anastomosis in group A was significantly longer than that in group B (P 0.05), but the time of venous anastomosis was less than that in group B (P 0.05). There was no significant difference in anastomosis time of partial bladder flap in ureteral band. Conclusion Orthotopic transplantation with end end anastomosis of renal arteriovenous end in rats can establish a stable and reliable animal model with fewer complications and faster healing. The success rate is similar, and the method of reserving the ligation line of renal vein during donor nephrectomy shortens the time of renal vein anastomosis.
