【摘要】：目的初步觀察自發(fā)性顱外段頸內(nèi)動(dòng)脈夾層腔內(nèi)治療的中期臨床療效及支架內(nèi)通暢情況。方法回顧性分析接受腔內(nèi)治療的自發(fā)性顱外段頸內(nèi)動(dòng)脈夾層患者臨床資料及影像學(xué)隨訪資料,對(duì)比分析腔內(nèi)治療前后及末次隨訪時(shí)美國國立衛(wèi)生研究院卒中量表(NIHSS)評(píng)分及支架內(nèi)通暢情況。結(jié)果2012年3月至12月共6例自發(fā)性顱外段頸內(nèi)動(dòng)脈夾層患者接受腔內(nèi)治療,其中男4例,女2例,中位年齡50歲(40.75~54.75歲),發(fā)病至接受腔內(nèi)治療中位時(shí)間為10 d(1周至3個(gè)月)。所有患者支架植入后夾層真腔血流立即恢復(fù),部分假腔內(nèi)仍有對(duì)比劑充盈;1例術(shù)中出現(xiàn)視網(wǎng)膜動(dòng)脈栓塞,無死亡患者。中位隨訪時(shí)間54.4個(gè)月(49.7~57.9個(gè)月),末次隨訪時(shí)NIHSS評(píng)分與術(shù)前相比,差異均無統(tǒng)計(jì)學(xué)意義(P=0.102);影像學(xué)復(fù)查可見頸內(nèi)動(dòng)脈支架段假腔均消失,支架內(nèi)無明顯狹窄。結(jié)論采用裸支架對(duì)部分有癥狀的自發(fā)性顱外段頸內(nèi)動(dòng)脈夾層進(jìn)行腔內(nèi)治療,可有效防止臨床癥狀復(fù)發(fā),隨訪發(fā)現(xiàn)夾層重塑良好并保持支架內(nèi)通暢。
[Abstract]:Objective to observe the mid-term clinical effect and stent patency of spontaneous extracranial internal carotid artery dissecting. Methods the clinical data and imaging follow-up data of spontaneous extracranial internal carotid artery dissection treated with intracavitary treatment were analyzed retrospectively. the (NIHSS) score and stent patency of the National Institutes of Health Stroke scale before and after intracavitary treatment and the last follow-up were compared and analyzed.
