【摘要】：目的探討替羅非班用于支架輔助動脈瘤栓塞的安全性及有效性。方法分析31例采用支架輔助治療動脈瘤患者的臨床資料,分為阿司匹林聯(lián)合氯吡格雷組(雙抗組,n=18)、替羅非班組(n=13)。替羅非班組在支架半釋放或完全釋放后開始使用替羅非班。術(shù)后隨訪評估患者有無新發(fā)梗死、支架通暢情況及動脈瘤栓塞效果。術(shù)前1天、術(shù)后3天分別檢測患者血小板計數(shù)及活化部分凝血活酶時間(APTT)。使用格拉斯哥預(yù)后評分(GOS)評價患者臨床預(yù)后,記錄術(shù)中、術(shù)后并發(fā)癥。結(jié)果 31例患者共發(fā)現(xiàn)動脈瘤34個,處理動脈瘤32個。術(shù)后隨訪動脈瘤栓塞后復(fù)發(fā)7例,泌尿系統(tǒng)出血1例,未出現(xiàn)腹膜后、胃腸道出血,未出現(xiàn)術(shù)后新發(fā)腦梗死及血栓栓塞事件。27例GOS評分≥4。兩組術(shù)前、術(shù)后血小板計數(shù)及APTT差異均無統(tǒng)計學(xué)意義(P均0.05)。結(jié)論替羅非班應(yīng)用于支架輔助動脈瘤栓塞安全、有效,緊急情況下可以作為雙抗的替代方案。
[Abstract]:Objective to evaluate the safety and efficacy of stent-assisted embolization of aneurysm with tirofiban. Methods the clinical data of 31 patients with aneurysm treated with stent-assisted therapy were analyzed and divided into aspirin combined clopidogrel group (double antibody group, n = 18) and tirofiban group (n = 13). The tirofiban group began to use tirofiban after half-release or complete release of the stent. Postoperative follow-up was performed to evaluate whether the patients had new infarction, stent patency and embolization effect. Platelet count and activated partial thromboplastin time (APTT).) were measured 1 day before and 3 days after operation. Glasgow outcome score (GOS) was used to evaluate the clinical prognosis of the patients. Results 34 aneurysms and 32 aneurysms were found in 31 patients. After operation, 7 cases recurred after embolization of aneurysm, 1 case had urinary hemorrhage, no retroperitoneal hemorrhage, gastrointestinal bleeding, and no new cerebral infarction and thromboembolism occurred after operation.
