【摘要】：目的：回顧性分析成人腦干膠質瘤的病例特點及臨床預后,研究影響成人腦干膠質瘤預后的相關因素。 方法：對2007年3月—2012年3月在中南大學湘雅二醫(yī)院被診斷為成人腦干膠質瘤患者52例患者診斷時的年齡、性別、診斷潛伏期、腫瘤分類、MR有無病灶對比度增強、是否手術及手術方式、病理分型、WHO分級、是否行放化療、生存時間等資料進行收集、整理。首先分析52例患者的總生存率及上述單因素與生存時間的關系,然后將單因素中與預后相關的及臨床上認為有意義的納入多因素回歸分析。 結果：1.在52例隨訪患者中,總的2年生存率為76.5%,中位生存年齡為61個月。2.單因素分析發(fā)現(xiàn)：52例患者診斷時的年齡在20-68歲之間(中位年齡31歲,大于40歲17例,小于40歲35例,)與患者的生存時間相關；診斷時的KPS評分(大于70分40例(76.9%),小于70分12例(23.1%))與生存時間相關；以及腫瘤的WHO分級(Ⅰ-Ⅱ級例37例,Ⅲ級9例,Ⅳ級6例)與患者生存時間相關。3.多因素COX回顧分析結果顯示腫瘤的WHO分級、診斷時的KPS評分與預后相關。 結論：1.診斷時的KPS評分與成人腦干膠質瘤的預后相關,KPS評分大于70分的患者預后更好。2.成人腦干膠質瘤的病例分級與患者預后相關,病理級別低的患者預后明顯好于病理級別高的患者。3.成人腦干膠質瘤患者診斷時的年齡可能與患者的預后相關,但需要更大樣本量、前瞻性的研究證實其相關性。
[Abstract]:Objective: to retrospectively analyze the characteristics and clinical prognosis of adult brainstem glioma and to study the related factors affecting the prognosis of adult brainstem glioma. Methods: from March 2007 to March 2012, 52 patients with adult brainstem glioma were diagnosed in Xiangya Hospital of Central South University. The age, sex, latent period of diagnosis, tumor classification and contrast enhancement of MR were studied. Whether the operation and operation mode, pathological classification, WHO grading, whether chemotherapy and radiotherapy, survival time and other data collected, collated. The overall survival rate of 52 patients and the relationship between the above univariate factors and survival time were analyzed at first, and then the prognostic factors were included in the multivariate regression analysis. The result is 1: 1. In 52 patients followed up, the overall 2-year survival rate was 76. 5% and the median survival age was 61 months. Univariate analysis showed that the diagnosis age of 52 patients was between 20 and 68 years old (median age was 31 years old, 17 cases were over 40 years old and 35 cases were younger than 40 years old).
